Abstract: A 60-year-old aunt Li repeatedly developed recurrent bilateral lower extremity sinkable edema in recent months, with symptoms such as low urination, poor food intake, poor night sleep, low urine volume, loose stools, pale tongue, white greasy coating, slow pulse, etc. Combined with the Chinese medicine diagnosis, the edema was considered to be caused by deficiency of spleen and Yang. The patient was treated with the medicine to strengthen the spleen and to promote the flow of water and dampness, which resulted in a better outcome and improvement of her condition. The patient was treated with a combination of the following drugs: Fu Ling, Atractylodes Macrocephala, Atractylodes Macrocephala, Radix et Rhizoma Atractylodes Macrocephala, Papaya, Cardamom, Ginger, Radix et Rhizoma Glycyrrhiza Uralensis, Radix et Rhizoma Ma, Ephedra, Radix et Rhizoma Chai Hu, Radix et Rhizoma Cuscutae, Radix et Rhizoma Lycii, Radix et Rhizoma Bacopa Monnieri, Radix et Rhizoma Chenpi, Radix et Rhizoma Shengdi. 4 weeks 【Treatment effect】 The patient’s edema basically subsided and her condition gradually improved. The patient had poor appetite, poor sleep at night, low urine volume, loose stools, pale tongue, white greasy coating, slow pulse, and other symptoms. He was admitted to the hospital with a Chinese medicine diagnosis of spleen-yang deficiency. After the patient’s informed consent, the patient was given treatment to warm the Yang and strengthen the spleen, and to promote the flow of water and resolve dampness. After 1 week of review, the patient’s urine output increased and swelling of the lower extremities was present, so the original prescription was continued. 3 weeks later, the patient developed fever, cough, light red tongue, thin white fur and floating pulse. Blood count: leukocytes 15.32×10^9/L, erythrocytes 2.88×10^9/L, lymphocytes 40%, neutrophils 93.1%. The main formula was supplemented with Ephedra and Chai Hu to increase the function of relieving the symptoms and promoting the lung, moving water and resolving dampness. After 4 weeks of standardized treatment, the patient’s swelling subsided and he slept peacefully at night, but was accompanied by weakness and poor appetite, so the original formula was supplemented with Radix et Rhizoma Ginseng, Semen Cuscutae, Fructus Lycii, Radix Bacopa Monnieri, Pericarpium Citri Reticulatae, and Radix et Rhizoma Lonicerae with water decoction to benefit Qi, warm Yang and strengthen the spleen. When the spleen and yang are not invigorated, water-dampness is not transported and the body swells, especially in the lower extremities. In this case, the swelling of both lower extremities was the first symptom, and the symptoms were spleen deficiency such as distension of the epigastrium and abdomen, poor appetite, loose stools, little urination, lack of color, and mental fatigue, etc. The pale tongue, white greasy moss, and slow pulse are signs of spleen-yang deficiency and internal accumulation of water-dampness, and the treatment was based on Fu Ling, Dagong Pi, Atractylodes Macrocephalae, Radix et Rhizoma Atractylodes, and Papaya to warm the yang and strengthen the spleen and resolve dampness. The patient’s condition finally improved after 4 weeks of treatment. The patient reported no further symptoms of spleen deficiency after one month of follow-up. The patient should be advised to strictly control sodium intake (≤5 g/d) and ensure balanced nutrition after discharge; try to eat more fresh fruits and vegetables, follow the principle of drinking less and eating more, eat more high potassium food, massage the edema at the bone ridge, and avoid strong movements such as pulling, pulling and pushing during massage; patients who are bedridden due to edema should keep Patients who are bedridden due to edema should keep the mattress dry and clean, and elevate the lower limbs at appropriate times to facilitate the decreasing of edema; for patients with severe edema, they can use air mattresses, turn over regularly and change the position to ensure proper movement of the affected limbs. For example, the patient in this article has severe edema of both feet, which is difficult to be restored by pressing on them, accompanied by symptoms of spleen qi deficiency such as reduction in nasal intake, loose urine and stools, and fatigue, etc. In order to take care of such edema of spleen yang deficiency, we should mainly eat less qi-producing foods, such as soybeans, sweet potatoes, peanuts, milk, etc., and take tonics mainly with thick decoction to avoid aggravating edema. Patients with vomiting due to peripheral edema can press acupuncture points such as Foot San Li, He Gu and Neiguan to relieve symptoms. For unexplained edema, it is imperative to go to the hospital to clarify the cause and carry out targeted treatment.
