Closed Liver Injury Grade

According to the 2018 revision of the American Association for the Surgery of Trauma grading method for hepatic trauma, there are 5 grades of closed hepatic injury levels. Grade I: hematoma: located under the peritoneum, <10% liver surface area; laceration: peritoneal tear, parenchymal laceration depth <1 cm. Grade II: hematoma: located under the periosteum, 10%-50% of the liver surface area; the diameter of the hematoma in the parenchyma is <10cm. Fissure: parenchymal fissure depth is 1~3cm, length is <10cm. Grade III: hematoma: subperitoneal, >50% of liver surface area or still expanding; ruptured subperitoneal or parenchymal hematoma: intraparenchymal hematoma >10 cm or still expanding. Laceration: depth >3cm. Grade IV: Fissure: parenchymal rupture involving 25% to 75% of the liver lobes or 1 to 3 Couinaud segments within a single lobe. Grade v: Laceration: parenchymal rupture involving more than 75% of the liver lobes or involvement of more than 3 Couinaud segments within a single liver lobe. The grading of liver trauma can guide the treatment and determine the prognosis, in case of liver injury you should go to a regular hospital for further diagnosis and treatment.
