What you need to know after sclerosing chocolate cysts (detailed version)

If you have recently undergone cyst sclerotherapy in our department, please read carefully to understand the following
After surgery you will need to watch for sudden onset of abdominal pain or fever, as well as other discomfort. Most patients have no significant postoperative discomfort. Please do not be alarmed if you notice any of the following
  1. If you find yourself with a slight increase in body temperature, don’t worry, the treatment of sclerotherapy will cause sterile inflammation of the cyst wall to promote adhesion of the cyst wall, there may be a slight fever, if the body temperature is not higher than 38.5 degrees Celsius, you can drink more water or physical cooling (warm water wipe, etc.). If the body temperature is higher than 38.5 degrees Celsius or you have fever-related discomfort, you can take oral ibuprofen and other drugs to reduce fever.
  2. If you find slight pain in the abdomen, you can first reduce your activities and rest flat on your back. Mild abdominal pain after surgery is a normal reaction. If there is severe abdominal pain, you need to contact our physician or go to the local hospital as soon as possible.
  3. If you have headache and weakness on the postoperative day or the second day after surgery, please do not worry too much because sclerosing agent is mostly highly concentrated alcohol, and there may be a small amount of absorption during the flushing process, and individual patients may have related near-drunkenness symptoms. The likelihood of this happening depends on the size of the cyst, the duration of sclerotherapy, and the amount of alcohol consumed by the patient. If your symptoms do not resolve or if you have a history of cardiovascular disease, please seek medical attention.
4.If you have other diseases that require medication, please consult the medication physician. The contraindication of medication can refer to the history of alcohol consumption on the operation day.
5.If you experience vague pain or discomfort in the pelvic or surgical area many days after surgery, you can contact our physician but there is no need to panic. Most patients have no significant postoperative discomfort in the operative area. If you have a feeling of deep constriction and vague pain in the operated area within 3-6 months after surgery, you can first review the ultrasound. If there is no obvious abnormality, it is usually considered that the contraction and pulling of the cystic wall around the sclerotic adhesions in the process of absorption and receding causes discomfort, which usually will gradually improve with the softening of the cystic wall adhesions after mechanization.
  6. Please do not perform too strenuous jumping or large torsional movements within 1 month after surgery. Because there is still exudate in the cyst during the exudative phase after the cyst has hardened, vigorous exercise may cause discomfort or torsion.
7.If you do not see the above discomfort, it is recommended that you come to our hospital for review 6 months after surgery.
If you have not been discharged, you can reflect your discomfort to the doctor in charge. If you have been discharged, you can contact the team doctor for consultation in the WeChat group.
  Common postoperative questions answered.
  1. Is it true that the cyst with poor sound transmission in the ovary was not cured during the early postoperative review? Is it a new occurrence?
It is recommended to review 6 months after surgery, and early review is not recommended if there is no discomfort. Usually, when a larger cyst is reviewed 3 months after surgery, you may still see the cyst that has not been completely absorbed, at this time, the fluid inside the cyst is exudate after sclerosis, and when the protein content is high, there will also be poor sound transmission, so it is generally recommended to review half a year. It is more accurate to review after six months after surgery.
2.Do I need to take short-acting contraceptive pills to prevent recurrence of chocolate cysts after hardening?
For cysts that have hardened, it is not necessary. However, it is worth noting that patients with deep pelvic pain or multiple chocolate cysts with smaller cysts that have not been treated can take the appropriate oral medication as prescribed by the doctor to prevent local recurrence or the growth of small cysts.
3.Will ovarian function be affected after surgery?
The sclerosing agent only acts inside the cyst cavity when the cyst is hardened, and the cyst is flushed with the sclerosing agent and then withdrawn, which has no effect on ovarian function. It is very rare to see a decrease in menstrual volume after surgery, and postponement of the first postoperative menstrual period has been reported occasionally.
  4. Postoperative spotting and bleeding
Postoperative hormonal changes may be caused by the pulling of the cyst or other operations, occasionally causing transient endometrial bleeding, which may appear as a small amount of light red or brown discharge. It is a normal phenomenon and does not need to be treated.
  5. Postoperative hematuria
Our puncture does not damage the bladder and other surrounding tissues, and surgery-related hematuria usually does not occur. If a change in urine color is found, it needs to be distinguished from spotting vaginal bleeding and communicated with the puncturing physician. If hematuria is diagnosed, combined urinary stones and other diseases cannot be ruled out, and urological consultation is required.
6.How long can you take a shower after surgery
You can take a shower after 48 hours after surgery, and be flexible according to the recovery of the puncture site. The puncture site can be covered with medical patch and then scrubbed before the puncture site is decabbed.
Finally, ultrasound-guided cyst sclerotherapy is an ultra-minimally invasive and effective treatment option with low risk and good efficacy, and is the first-line option for cyst treatment.
