The most common surgical procedure for entropion is orbicularis oculi shortening, which is performed as follows: the patient is placed supine on the operating table, routinely disinfected, head wrapped, and a sterile cavity towel is placed, and the lower lid skin is anesthetized with a local infiltration of 2% lidocaine injection 0.5 ml. A skin incision parallel to the lid margin is made 3 mm from the lower lid margin, approximately 2.5 cm in length, and the subcutaneous tissue is separated to identify the orbicularis oculi muscle. A small strip of orbicularis oris muscle is separated and a set of mattress sutures is made, the orbicularis oris muscle is folded, shortened by approximately 8 mm, and the excess muscle is cut. The eyelid is observed, excess skin of the lower lid is cut away, and 5-0 sutures are interrupted to close the eyelid skin. Again the area is cleaned and disinfected, tobramycin dexamethasone eye ointment is applied, and the surgical eye is bandaged.
