Although there are various reasons for the formation of stroke, the basic pathogenesis is always a disorder of yin and yang and a rebellion of qi and blood. The location of the disease is in the heart and brain, and is closely related to the liver and kidney. The Suwen Pulse Essentials Essence says: “The head is the house of the essence.” Li Shizhen also pointed out in the “Compendium of Materia Medica” that the brain is the “House of the Essence”. Both “Jingming” and “Yuan Shen” refer to the function of mastering mental consciousness and thinking activities, so it can be considered that Shenming is the master of the mind and brain. The pathological basis is Yin deficiency of the liver and kidney. The deficiency of liver and kidney yin makes it easy for liver yang to become hyperactive, and when combined with improper diet and living, emotional stimulation or external evil, the qi and blood rush up to the brain and the nerves are blocked, so the patient suddenly faints and becomes unconscious. The pathological factors are mainly wind, fire, phlegm and stasis, and their formation is related to the dysfunction of the internal organs. For example, liver and kidney yin deficiency, hyperactivity of yang to fire and wind, or five will to fire and wind. The spleen is not healthy, phlegm is generated, or fire and heat refine liquid into phlegm. The stagnation of blood stasis can be caused by anger and blood aster on the top, or Qi deficiency and inability to push. The four can affect each other or see the same disease, such as wind and fire incite each other, phlegm and blood stagnation and so on. In severe cases, wind, Yang, phlegm, fire and qi and blood block the brain and run across the meridians, resulting in fainting, aphasia, and dysarthria. The nature of the pathology is mostly deficiency of the root and the symptoms. Liver and kidney yin deficiency, qi and blood weakness is the origin of the disease, wind, fire, phlegm, qi and stasis are the symptoms of the disease, and they can be mutually causal. At the beginning of the disease, the evil is scops, the wind, Yang, phlegm and fire are strong, and the Qi and blood are on the top, so the standard is mainly real; if the disease changes drastically, under the fierce attack of the evil, the positive qi collapses rapidly, so the positive deficiency can be the main cause, and even the positive deficiency occurs. In the later stage, because the positive qi is not restored and the evil qi is left alone, it can leave sequelae. Due to the difference in the depth of the disease and the severity of the disease, there are differences between the meridians and the internal organs. The lighter ones are in the meridians and the heavier ones are in the internal organs. If the liver wind and phlegm are in the meridians, the blood vessels are stagnant, and the qi and blood cannot moisten the body, the evidence of the meridians is seen, manifesting as hemiplegia, distortion of the mouth and eyes, not accompanied by mental disorders; if the wind and phlegm fire obscure the orifices, the qi and blood reverses and rushes up to the brain, the evidence of the visceral organs is seen, with the loss of blood overflow. Stasis blocking the brain, resulting in sudden fainting and unconsciousness. Due to the difference in the deficiency of the evil, there is the evolution of closed and off, closed evidence in the internal organs, due to hyperactivity of liver yang or phlegm-heat internal organs, the wind and phlegm up disturbance, see the Deng Shuo, the spirit is not clear, the stool does not pass; in the organs, the wind and Yang phlegm fire within the closed orifices, brain complex stasis, then see fainting, unconsciousness, limb detention and other closed evidence. If it is due to phlegm and fire and heat stasis, it is Yang closed; if it is due to phlegm and cloudy stasis, it is Yin closed. If the wind and yang phlegm and fire are hot, further consuming the yin essence, yin deficiency and yang, yin exhaustion and yang death, yin and yang leaving the decision, then there will be a loss of evidence, manifested as mouth opening and eye closing, hand spreading and limb cold, weak breath and other symptoms of deficiency. It can be seen that although the pathogenesis of stroke is complex, it can be summarized as deficiency (yin deficiency, blood deficiency), fire (liver fire, heart fire), wind (liver wind, external wind), phlegm (wind phlegm, damp phlegm), qi (qi reversal, qi stagnation), and blood (blood stasis). The recovery period is due to the loss of Qi and Blood, and the blood vessels are not smooth and the posterior meridian form evidence. However, due to the deficiency of liver and kidney yin, the loss of qi and blood has not been restored, and the evil of wind, fire, phlegm and blood stagnation stagnates in the meridians, the qi and blood does not run smoothly, and there are still sequelae such as hemiplegia, mouth distortion or incoherence, and it is generally difficult to recover.
