The causes of high HDL cholesterol are physiologic and pathologic. For example, people who exercise regularly have high HDL cholesterol. While people who exercise less and those who smoke regularly may have low HDL cholesterol.
Pathological factors causing elevated HDL cholesterol include chronic hepatitis and patients with primary biliary cholangitis, which are caused by cholesterol transferase deficiency, leading to reduced hepatic lipase activity and thus causing high HDL cholesterol.
