Clinical application of 2 micron laser

  The 2 micron laser is imported from Germany and has a wavelength of 2.013 μm and a maximum power of 120 W. It combines the advantages of the green laser “vaporization” method and the holmium laser “cutting” technology. The penetration depth of the 2 micron laser in the tissue is only 0.3 mm, and the coagulation layer left after removing the hyperplasia is only 1 mm. It is fast, bleeds less, and removes more tissue.  Clinical applications: prostate hyperplasia; bladder tumors; urethral strictures; ureteral strictures; bladder diverticula; open surgery The advantages of the 2-micron laser include simultaneous “vaporization” and “precision cutting” procedures. When the laser hits the tissue, the powerful energy is instantly absorbed by the water, producing a strong effect that vaporizes and cuts the tissue, and the surgical wound is immediately coagulated and hemostatic, shortening the surgical time significantly.1. For patients with prostatic hyperplasia transurethral resection of the prostate (TURP) is recognized as the gold standard for surgical treatment of BPH (prostatic hyperplasia), but TURP still has shortcomings, the most important of which are Electroresection syndrome (TURS), bleeding, and severe TURS even leads to death.  Compared with TURP: 1, the improvement of subjective and objective symptoms and reoperation rate are comparable; 2, the retention time of catheter and hospitalization days are shorter than TURP 3, less bleeding, no need for blood transfusion, clearer intraoperative visualization 4, no occurrence of water intoxication 5, can be used for patients with anticoagulation 6, low incidence of retrograde ejaculation 7, easier to master.  For patients with bladder tumor compared with transurethral resection of bladder tumor (TURBT): 1. no closed nerve reflex 2. complete tumor enucleation 3. can reach deep into the muscular layer and accurate postoperative staging 4. almost no bleeding 5. no need to stop anticoagulants such as aspirin before surgery. Our hospital introduced German imported thulium laser urological system from 2014, which is the first of its kind in the whole western region of Guangdong. At present, no complications such as urinary incontinence and postoperative recurrence have occurred in many cases treated with this technology, which marks that the application of this technology in our hospital has reached the international leading level.
