Introduction to the modalities and mechanisms of surgical treatment of type 2 diabetes

Many people have the perception that diabetes can only be treated by diet control and medication. In fact, as early as the “China Type 2 Diabetes Prevention and Treatment Guidelines”, it is recognized that type 2 diabetes can be effectively treated through weight loss surgical procedures such as gastric bypass surgery and sleeve gastrectomy. The surgical treatment of diabetes has a history of nearly forty-one years, and this surgery has become very mature abroad. The surgical treatment of diabetes was an accidental discovery by the surgeon, who initially reduced the stomach volume of the obese patient for cosmetic weight loss and made a small adjustment to the shape of the intestine. The surgeon was very excited. The patient’s blood sugar dropped significantly in the weeks following the surgery. This led to the development of the gastric bypass surgery that has matured in recent years through continuous refinement of the procedure. Since 1994, when the world’s first laparoscopic gastric bypass surgery was performed, laparoscopic gastric bypass has become the gold standard in bariatric metabolic surgery and the only treatment for patients with obesity 2 diabetes. Surgical approach: The intestinal structure is altered to reduce the space of the stomach and the length of the small intestine by displacing most of the gastric function. On the one hand, a gastric bursa (15-30 ml) is built in the upper part of the stomach to limit food intake; on the other hand, the distal jejunum is spared through the best absorbing duodenum and the first section of jejunum is anastomosed with the gastric bursa, thus greatly controlling food absorption. Surgical features: high safety, low pain, low bleeding, fast recovery; high effectiveness, low complications; reversible digestive tract structure Therapeutic mechanism: endocrine changes, improved restoration of islet cell function Reduced dietary intake and absorption, reduced glycemic load Weight loss, increased insulin sensitivity Change in intestinal flora to reduce blood glucose Compared with traditional medical drug treatment, today’s weight loss surgery has 2 main advantages First, the treatment effect is significantly better than internal medicine. The treatment effect is significantly better than internal medicine. According to Wu Liangping’s more than 10 years of experience in bariatric metabolic surgery and more than 1,200 surgical cases, the long-term complete remission rate of diabetes reached more than 80% after sugar patients underwent bariatric metabolic surgery, and diabetic patients not only got rid of taking medication and insulin to control blood sugar, but also stopped the process of diabetes and even reversed some of the complications of diabetes. The second advantage is the short treatment time. Traditional internal medicine is a lifelong treatment, which requires a lot of labor and material resources, while the hospitalization period of weight reduction surgery is about 7 days, and the surgery time is 60 minutes-90 minutes, plus because of the minimally invasive technology, the patient recovers quickly and can get out of bed and walk around the day after the surgery, and can live and work normally after being discharged.
