Patients who come to the pain physiotherapy department often ask the same question, “Doctor, what is causing my pain? What method of treatment should I use?” This question is complex and simple. It is complicated because pain is a common manifestation of many diseases, and different diseases that can cause pain have different etiology and pathogenesis, and there are even many diseases whose etiology and pathogenesis cannot be explained clearly yet. There are many ways to treat pain in Western medicine, and the use of pain medication is one of the major treasures of Western medicine. However, the side effects are so great that patients are often discouraged. If we understand the etiology of pain from the perspective of Chinese medicine, the problem becomes simpler. The ancient word “pain” is very wonderful. In the word “pain” is winter, and winter is winter. According to Chinese medicine, the Qi of winter is cold and its form is water. Therefore, from this character we can see that pain is closely related to cold and water. Without cold and water, pain will not be formed. This also points precisely to the main cause of pain. As early as 2,500 years ago, there was a treasured book that was followed by generations of Chinese doctors, and it was the Yellow Emperor’s Classic of Internal Medicine. This book covers the physiology of the human body, pathology, diagnosis and treatment of diseases, as well as preventive health care and many other topics. In this book, which is known as the “health bible” of the East, there is a special article on pain called “Treatise on Pain”. In this article, there are 14 examples of pain, 13 of which are caused by cold, and only one is caused by heat. This is a good example of the close relationship between pain and cold. Let’s take a look at the word “pain”, which has the character “甬” (sound symbol) inside it, and Shuo Wen Jie Zi says: “Yong, the road is also.” In the word “Yong” outside of the character “disease” side, which is a very graphic description of the road problems. As we all know, the road is used for walking, traffic, and now the road has problems, naturally is not pass. When it does not pass, there will be pain. This is what Chinese medicine calls “pain is not passable, passable is not painful.” As a TCM practitioner, we usually consider and choose between these two causes of pain and determine the treatment method. In our pain physiotherapy department, we use acupuncture to treat various painful diseases, and we have very good results. Why is acupuncture able to reduce pain? Studies have shown that acupuncture points can induce the release of biologically active chemicals such as 5-hydroxytryptamine, endorphins, and prednisolone in many parts of the central nervous system, which accelerate the clearance of inflammatory and pain-causing substances, thus producing an analgesic effect.
