It is generally accepted that medication reduction until complete discontinuation of antiepileptic drugs can be initiated 3-4 years after seizure cessation. Even so, the recurrence rate after withdrawal is generally around 30%. The relapse rate is especially high within 3-6 months of withdrawal. There are many reasons for relapse, including: 1. Self-discontinuation or too rapid a reduction in dosage, usually after 1-2 years from the start of the drug reduction to discontinuation, otherwise relapse is likely to occur. 2. A high relapse rate of about 50% after discontinuation in patients with onset of seizures after age 30. 3. The patient has a family history of epilepsy. 8. has a rhythmical change in the EEG. 9. has a high blood level before discontinuation of medication.
