I. Initial consultation
The patient, a 68-year-old female, complained of dizziness and slight eye distention a month ago. The patient’s eye examination showed a greenish-gray reflection in the pupil area, along with iris swelling, an unclear fundus, turbid congestion in the bulbar conjunctiva, and a mildly dilated pupil. The IOP was measured at 60 mmHg, and the IOP was elevated. The patient was initially diagnosed as having an acute attack of angle-closure glaucoma and was treated immediately.
II. Treatment process
After informing the patient of the examination results and diagnosis, the patient expressed positive cooperation with the treatment, so the patient was given intravenous rapid infusion of mannitol injection and topical application of trigonelline nitrate eye drops to reduce IOP, while the patient was given oral treatment with vinpocetine tablets, which can effectively inhibit carbonic anhydrase in the patient’s ciliary body, thus reducing the formation of water resistance, synergistically reducing IOP and relieving the symptoms of glaucoma, and the patient was instructed to The patient was instructed to repeat the examination after 1 week.
III. Treatment effect
The patient was re-examined 1 week later and said that the symptoms of blurred vision had basically disappeared, and the symptoms of eye pain and congestion had significantly improved, and no adverse reactions occurred during the medication.
IV. Notes

V. Personal insight
Early symptoms of closed-angle glaucoma are usually mild, such as this patient, which may only be accompanied by mild dizziness and slight eye distention, but without timely treatment, it may easily lead to further aggravation of the disease, and then symptoms such as eye distention and pain, bulbar conjunctival congestion and vision loss. Therefore, it is recommended that when you feel the occurrence of unexplained adverse symptoms in your body, you should go to the hospital promptly to avoid delaying the condition, which may lead to aggravation.
