The Yellow Emperor’s Classic of Internal Medicine, which laid the foundation of Chinese medicine more than 2,000 years ago, tells us about the physiopathology of sleep and explains the difference between the elderly and the young in terms of sleep from the perspective of human physiology, which we will study today. In the chapter of “Nei Jing – Ling Shu – Ying San Hui”, it is said that “the old man who does not close at night, what is the gas that makes him do so? The young and the strong do not sleep at night, what is the gas that makes it happen?” It means that the old people can’t sleep at night, what is the reason for this? Young people do not sleep in the daytime, why is this? Later, Qibo said: the strong man’s qi and blood is strong, his muscles are smooth, the qi channel is open, the movement of the camp and guard does not lose its normal, so the day essence and night close. This Qibo is Huang Di’s teacher, he replied: young and strong people are full of qi and blood, their muscles are smooth, the meridian paths are open, the Ying Qi and Wei Qi each have their own duties, each follow their own paths, so the daytime spirit and sleep at night. “The old man’s qi and blood decay, their muscles wither, qi paths are astringent, the five organs of qi fight each other, their camp qi decay less and the guard qi within the felling, so the day is not fine and the night is not close.” This sentence explains that as the elderly grow older, their qi and blood become weakened, their muscles dry up, their qi and blood flow paths are blocked due to various chronic diseases, their qi and blood flow is not smooth, their five organs are not coordinated, and their ying qi is weakened, so they are not energetic during the day and cannot sleep at night. Therefore, Nei Jing gives us an explanation of the etiological factors of insomnia, the basic cause of insomnia in the elderly is because of the ageing, the weakness and decline of Qi and blood, and the elderly are more likely to get insomnia if they have underlying diseases, and the blockage of the veins and channels of Qi is also an important reason for the onset of insomnia in the elderly, so when we identify and treat insomnia in the elderly, we consider the patient’s weakness of Qi and blood on the basis of evidence-based treatment, and at the same time activate the blood to remove blood stasis.
