Clinical manifestations of itchy rash of pregnancy

  1. History: (1) Most often seen in women with 2 or more pregnancies with sudden cholestasis of pregnancy jaundice.  (2) It resolves spontaneously within 3 weeks after delivery. It can occur later in pregnancy.  (3) Early-onset rash occurs within six months of pregnancy, mostly in the third or fourth week of pregnancy; late-onset rash occurs in the last two months of pregnancy, especially in the two weeks before delivery.  (4) Early-onset lesions usually occur on the upper part of the trunk, upper arms and femur, more often on the extensor side, symmetrical on both sides, and occasionally on the abdomen and buttocks; late-onset lesions first occur on the abdomen at the stretch marks, and can spread rapidly throughout the body.  (5) Intense pruritus, especially at night.  (6) In severe cases, stillbirth may occur.  2, the clinical manifestations of pregnancy itchy rash ① early type of pregnancy itchy rash: mostly seen after 3 to 4 months of pregnancy. The rash is often symmetrically distributed on the trunk, proximal extremities, abdomen and buttocks, is a light red papular rash, about 0-3 cm in diameter, itching is more intense, there may be scratch marks, blood crust or epidermal peeling.  ②Late onset pregnancy itch rash: common in the first 2 months to 2 weeks before delivery. The rash mostly occurs on the stretch marks and gradually develops all over the body. In addition to the early type of damage (papular rash), some may present as a rash of wind clusters and erythema multiforme-like rash. Itching is intense, there may be scratch marks, blood crust and skin hypertrophy, and even moss-like changes.  3, treatment: ① internal medication for pregnancy itchy rash: the same as itchy rash. Symptomatic antipruritic and anti-allergic treatment, can be internal antihistamines, sedative sleeping agents, etc.. For severe cases, a short course of corticosteroids (small doses of prednisone tablets) can be used internally as appropriate.  ② Topical medication: topical application of furnace glycerin lotion or comfrey oil.
