Normal, healthy newborns require vaccinations on the first day of life, including BCG vaccine, which is given to the deltoid muscle of the left upper arm as a vaccine against tuberculosis. Since BCG is a live attenuated vaccine and produces a delayed metaplasia, an inflammatory reaction of redness, swelling, and pus will occur about 4 weeks after vaccination. If the inflammation is mild, no special treatment is needed. Avoid getting water on the area and prevent secondary infection. If the local inflammatory reaction is strong in individual children, topical isoniazid ointment or rifampicin ointment is needed, and if necessary, surgical consultation for incision and drainage is required.
