First, testicular infection or epididymitis, when pain in the testicle or epididymis can occur, accompanied by obvious swelling. Sometimes the testicle and epididymis are so closely adhered that it is not easy to distinguish them. In acute inflammation, reactions such as redness and swelling of the scrotal skin can be seen, and the inflammation involves the spermatic cord, so pain can be radiated to the lower abdomen, especially after activity. Blood tests and ultrasound examination of the scrotum are needed to clarify the diagnosis. After the diagnosis, anti-infection treatment should be actively given to reduce the symptoms. Secondly, right testicular torsion, when acute ischemia of the testicle occurs, there is obvious pain. It also involves the spermatic cord and can radiate to the pain in the small abdomen. The pain gradually worsens with time. An ultrasound examination of the scrotum and spermatic cord is needed immediately to rule out the presence of torsion. If testicular torsion is present, immediate surgical repositioning is required.
