How to Rehabilitate Thoracolumbar Scoliosis

Rehabilitation training for thoracolumbar scoliosis includes longitudinal extension, inversion exercises, muscle endurance exercises under corrected position, rotational breathing, and corrected daily posture training.
1. Longitudinal extension: the patient’s muscles exert themselves to lift the trunk as far as possible, keeping the pelvis in the correct position during the process.
2. Reversal exercises: designed to correct rotational deformities. Since the convex side of scoliosis bulges dorsally, rotation should be corrected by rotating the convex side ventrally. If the thoracic segment is bent to the right in the coronal plane, a corrective force is applied to the thoracic segment toward the ventral side, and a corrective force is applied to the lower lumbar pelvis toward the dorsal side.
3. Corrective Subluxation Muscle Endurance Exercises: Maximum loads are used in active self-correction so that the patient performs an active self-corrective maneuver and maintains it throughout the isometric contraction to improve muscular endurance in the paraspinal, abdominal, and lower lumbar segments and around the scapulae.
4. Rotational Breathing: The patient selectively contracts the convex side of the trunk so that the inhaled air reaches the concave side of the lungs. During inhalation, the convex side of the thoracic segment is rotated ventrally and force is applied to pull up the tissue on the concave side of the trunk through the rotation of the vertebrae in order to direct the breath into the lungs on the concave side of the trunk.
5. Corrective daily postural exercises: increase the anterior convexity of the lumbar spine when standing, tilt the pelvis to the other side of the original lateral tilt, place the hand on the waist to correct the curvature of the lumbar region, place the other hand on the arch of the rib cage to correct the rib cage position, and take a catwalk when walking.
