Myelofibrosis can improve the related symptoms through standardized treatment and cannot be completely cured. The treatment methods include medication, surgery, radiation therapy and so on.
1. Drug therapy: commonly used drugs include drugs for spleen enlargement, drugs for anemia, androgens, erythropoietin, immunomodulators, vitamin D3, chemotherapeutic drugs and so on.
(1) Drugs for treating splenomegaly: for example, JAK2 inhibitors combined with thalidomide are used to reduce the symptoms of splenomegaly and improve hematopoietic abnormalities.
(2) Drugs for treating anemia: for example, iron and folic acid are used to replenish the body’s lack of hematopoietic substances and improve the symptoms of anemia.
(3) Androgens: Danazol, testosterone propionate, etc., used to improve the hematopoietic function of bone marrow.
(4) Erythropoietin: used to treat anemia.
(5) Vitamin D3: to improve anemia and inhibit disease progression.
(6) Chemotherapeutic drugs: such as vincristine, prednisone, etc., used to inhibit the progress of myelofibrosis.
2. Surgical treatment
(1) Splenectomy: to improve the symptoms of splenic hyperfunction and splenomegaly compressing the surrounding tissues.
(2) Allogeneic hematopoietic stem cell transplantation: to remove the immune system through radiotherapy and rebuild the recipient’s hematopoietic and immune systems.
(3) Transjugular intrahepatic portosystemic shunt: aims to reduce portal hypertension.
3. Radiation therapy: also known as radiotherapy, to shrink the spleen through localized spleen radiation therapy.
