How to rehabilitate stroke patients

In the clinical treatment, I often encounter patients or family members to consult, that is, after suffering a stroke, although the hospital timely rescue, life is saved, but half of the limb mobility is significantly reduced, such as walking, holding things, brain response and memory are significantly different than before the disease, as if the hands and feet do not obey, joint activity is not flexible, walking is also very hard, at first, many patients are At first, many patients thought that it was due to the weakness of the body after a serious illness, and slowly wait for the recovery period to pass and can heal on their own, but after resting at home for several months, the condition of the functional activities of the limbs not only did not improve, but, on the contrary, seemed to be worse than when they were discharged from the hospital. How should a stroke patient undergo rehabilitation treatment? As we all know, stroke is a disease in which the patient’s brain or nerves are violated due to ischemia (including cerebral infarction and cerebral embolism) or hemorrhage, which affects a series of functions of the patient’s body, such as movement, sensation and speech. If the function is slightly impaired, the patient can recover on his own; if the function is moderately or severely impaired, most of the patients cannot recover on their own, and these patients must undergo early and comprehensive rehabilitation treatment under the collaborative treatment of multidisciplinary doctors in order to maximize the recovery of lost functions: if the limb function is severely impaired, acupuncture, massage, biofeedback therapy, etc. must be carried out under the guidance of professional doctors. If the functional activity of the limbs is severely impaired, a combination of Chinese and Western medicine rehabilitation treatments such as acupuncture, massage, biofeedback therapy, and exercise therapy must be performed under the guidance of a professional doctor. If the patient has difficulty in speaking or swallowing, or cannot understand others’ speech, he/she should use acupuncture, ice stimulation, electrical stimulation, etc., and actively cooperate with tongue movement training under the guidance of a rehabilitation therapist; if the patient has reduced sensory function of the affected limb, he/she should protect the affected limb to avoid accidental injury to the affected limb; if the limb is painful, he/she should ask a professional doctor to find the cause of the pain and use acupuncture In the early stages of stroke, rehabilitation treatment with Chinese and Western medicine should be carried out in the hospital as far as possible, especially noteworthy is the pivotal role of acupuncture in the rehabilitation of stroke. Decades of clinical practice have shown that, under the premise of stable disease, the early or late intervention of acupuncture, or the absence of acupuncture treatment directly affects the patient’s rehabilitation efficacy, and also significantly affects the amount and degree of residual disease. How to carry out scientific and effective rehabilitation treatment, this is a constantly developing, constantly updated understanding of the problem. The author believes that, in general, the rehabilitation treatment of stroke disease should strictly follow the principles of “timely treatment, early rehabilitation, and combination of Chinese and Western medicine”. If the rehabilitation can be carried out in time, the possibility of recovery is the greatest, after six months the possibility of recovery gradually becomes smaller, and after one year it is generally difficult to recover significantly. One view is that the treatment of stroke disease should be based on medical treatment, and rehabilitation is optional, depending on the family’s economic conditions. At present, fewer and fewer people hold this view, because with the continuous improvement of the people’s knowledge level, people’s awareness of stroke disease has greatly improved, and everyone has fully realized that now that medical conditions are better, saving the lives of stroke patients has become extremely easy, and the biggest medical problem at present is to minimize the sequelae of patients; another view is: the acute period can not be carried out The other view is: the acute period can not be rehabilitation training, the recovery period at home to move or family members to help activities on the line. However, with the continuous development and progress of rehabilitation medicine, practice shows that the acute phase of rehabilitation is very important, especially the Chinese medicine of acupuncture, as an important part of the rehabilitation therapy, if early scientific and effective rehabilitation treatment, it will affect the patient’s future function The degree of recovery and quality of life. In Europe and the United States, where medicine is very advanced, the combination of Chinese and Western medicine in rehabilitation therapy has been highly valued and widely used. Generally speaking, acupuncture should be considered as soon as the patient’s vital signs are stable, about 1-2 weeks after the onset of stroke due to cerebral hemorrhage and 2-3 days after the onset of cerebral infarction. The earlier the patient receives acupuncture treatment, the greater the patient’s hope of recovery, and the lighter and fewer the sequelae will be. Many people believe that acupuncture treatment for cerebrovascular disease should be done after the acute phase, avoiding the period of cerebral shock, which is highly undesirable and will only cause the patient to miss the best time for recovery. The acute and early recovery phases of stroke are critical periods for implementing rehabilitation treatment and affecting prognosis, and acupuncture and rehabilitation training should intervene early. Acupuncture is effective for the rehabilitation of patients in all three phases, especially for patients in the acute phase and early recovery. Acupuncture has a long history of treating stroke disease with unique efficacy. In the classic “Huangdi Neijing”, a classic of Chinese medicine more than 2000 years ago, there is a clear record: “partial withering (i.e. hemiplegia), the body is partial without using and pain, the words remain unchanged, the will is not confused, the disease is between the points of the couples, the giant needle to take, benefit its deficiency, loss of its surplus, can be restored.” After generations of doctors on the treatment of acupuncture and moxibustion have been discussed, whether to take points or acupuncture methods, have been constantly improved and improved, and accumulated a wealth of experience. Modern acupuncturists, based on the experience of their predecessors and using modern scientific methods, have continued to innovate, so that the efficiency and cure rate of acupuncture for this disease have been greatly improved. The traditional acupuncture method mainly focuses on unblocking the meridians, mostly taking Yang meridian points, supplemented by Yin meridian points, which is really effective for most of the symptoms of stroke paralysis. Recently, the acupuncture method of “awakening the brain and opening the orifices” has been applied in the clinical treatment of stroke disease. The method has been innovated in the selection of acupuncture points and the operation of acupuncture techniques, mainly taking yin meridian points and supplementing with yang meridian points, mainly to improve the function of the brain, which is the capital of the spirit, to awaken the spirit and open the orifices, and to activate the blood and open the channels. The acupuncturist of the stroke disease specialist also introduced that as early as 2007, the stroke disease specialist of Jiaxing City Hospital was the first to carry out scalp acupuncture + “awaken the brain and open the orifice acupuncture method” in the treatment of stroke disease in Jiaxing area and achieved satisfactory results, which greatly reduced the mortality, disability and recurrence rates, and strongly improved the clinical efficacy and gave new hope to the recovery of stroke patients. The author is of the opinion that the treatment of stroke disease is a new hope. In the author’s opinion, the rehabilitation treatment of stroke can be intensively trained in several time periods of one month, three months, six months and one year after the disease. In a word, the earlier the intervention of rehabilitation therapy, the smaller the scope of brain damage, and the more scientific, reasonable, and combined Chinese and Western medicine treatment can enable stroke patients to establish cerebrovascular collateral circulation early, improve the blood supply to brain tissue, and thus help the body to recover its functions. On the contrary, late or unscientific treatment will definitely delay the patient’s chance of recovery, and if the brain tissue at the lesion site is already calcified, it will be difficult to recover the damaged neurological function.
