Five years after radical mastectomy for breast cancer and one year after hysterectomy and bilateral adnexal resection for left ovarian cyst, the patient was found to have a 0.3-cm diameter strongly echogenic cluster in the posterior wall of the gallbladder without posterior acoustic shadowing that did not move with body position during a regular physical examination at a hospital in Beijing, where the first two surgeries were performed, in October 1995. It was diagnosed as a gallbladder polyp. Surgery was ordered when it grew to a diameter of 1 cm or more. In mid-April 1996, when the ultrasound was repeated, the original gallbladder polyp was found to have grown to 0.6 cm in diameter, and because of the rapid increase in polyp size, the patient was suspicious and afraid of further surgery. Present symptoms: fullness in both hypochondria, vague pain in the right hypochondrium, depression, frequent sighing, dull red tongue, white coating with roots, and slow pulse. The patient was diagnosed with dysmenorrhea. Herbs: Radix Bupleurum, Radix Paeoniae Alba, Radix Rehmanniae Praeparata, Radix Angelicae Sinensis, Rhizoma Ligustici, Rhizoma Ligustici, Rhizoma Polygonati, Rhizoma Polygonati, Radix Safflamiae, Rhizoma Citrus Reticulatae each 12g, Rhizoma Urticae, Rhizoma Dioscoreae each 15g, Rhizoma Curcumae, Rhizoma Oyster each 30g, Radix Glycyrrhiza Urticae 6g. After taking 49 doses of the original formula of angelica to 20g and roasted licorice to 10g, the patient was reviewed at our hospital and the former Beijing Surgical Hospital, and both confirmed that the original gallbladder polyp had disappeared. According to gallbladder polyps is a general term for various benign elevations of gallbladder mucosa, modern medicine advocates surgical treatment. In this case, although the type of gallbladder polyp could not be determined and its diameter did not reach 1.0 cm, it grew faster and had conscious symptoms, and in view of the patient’s history of two tumor surgeries, gallbladder removal became an imperative treatment option sooner or later. According to traditional Chinese medicine, regardless of the type, gallbladder polyps are always phlegm, stagnation and stasis in the gallbladder, resulting in stagnation of qi and blood, and the accumulation of phlegm and stagnation over time, which is similar to rotting flesh. The patient has suffered two major surgeries in a row and still has persistent illnesses, depression, poor qi flow, and stagnation of phlegm and stasis, resulting in loss of drainage of the liver and gallbladder, resulting in pain and distension in the dorsal region. The formula used is Blood Mansions and Blood Stasis Tang with Addition and Reduction. The formula contains Sichiban, which dredges the liver and gallbladder and regulates the Qi flow; Peach Red and Four Objects Tang and Curcuma longa, which nourish the blood and invigorate the blood, resolve blood stasis and disperse knots; raw oyster and stiff silkworm, which soften and resolve phlegm; and Ume plum, which leads the medicine into the liver and gallbladder, which is said to “erode evil flesh” in Ben Jing. The whole formula dredges the liver and gallbladder, resolves blood stasis, removes phlegm, softens hardness and disperses knots. After the tongue was dissolved, the dosage of angelica and licorice was increased to nourish the blood and protect the righteousness, so that although the agent of dispelling evil was taken for a long time without harming the righteousness, the stubborn disease was finally cured and the pain of surgery was spared.
