Principles of treatment for gallbladder polyps?

Previously, the principles of treatment of gallbladder polyps belonged to the logic of speculation, estimation, possibility, and taking things for granted; either extreme left – it is better to kill three thousand by mistake than to release one, and to remove all gallbladders. Either extreme right – those with stones under 10 mm in diameter have a high probability of safety, and conservative observation is performed. These methods are based on vague concepts that are not scientific. It is not reasonable and it is wrong. Nowadays, in the new era of high technology, with direct endoscopic observation; electron microscopy, three-dimensional imaging, cell cloning and other technologies, those “eternal foci”, “hotbed theory”, “gastric ulcer must be removed The “eternal foci”, “hotbed theory”, “gastric ulcer must be removed”, breast “lump” must be removed and other unchanging theory, it seems inappropriate. The court mainly relies on evidence, and the main basis for judging whether the gallbladder lesion is benign or malignant should be the pathology report, other methods are speculation, estimation, imagination or taking for granted, are not scientific. Only according to the pathology report is correct. Nowadays, the “endoscopic minimally invasive biliary polyp removal” method carried out by Peking University First Hospital Endoscopic Surgery and Peking University Shougang Hospital is to remove the polyps one by one through small percutaneous incisions in the gallbladder under the direct view of the choledochoscope, and send them to frozen pathological examination immediately. If it is malignant, radical resection of gallbladder cancer is performed; if it is benign, the gallbladder is closed and preserved with minimal damage, which is very reasonable. It is very popular among patients. This method has been awarded the Beijing Science and Technology Achievement Award and the Metallurgical Ministry Science and Technology Achievement Award. Don’t move to remove the gallbladder! Modern technology has discovered that the gallbladder is an important digestive organ of the human body, with complex digestive and immune functions, and more importantly, plays a very important role in maintaining the pressure balance in the bile duct. The size of the surgical incision cannot be used as a criterion to distinguish minimally invasive surgery; therefore, any so-called minimally invasive surgery to remove the gallbladder cannot be called “minimally invasive surgery”! Because the removal of the gallbladder itself has lost the function of the gallbladder, how can we talk about minimally invasive surgery on the gallbladder? It’s a major invasion! In the era of minimally invasive medicine in the 21st century, this is especially important.
