Introduction and treatment of pemphigus vulgaris

 
Alopecia areata, commonly known as “ghost shaving”, is a sudden onset of limited patchy hair loss. The scalp is normal and there is no inflammation or self-perceived symptoms. The course of the disease is slow and can remit and recur on its own. If the entire scalp loses all its hair, it is called alopecia totalis. If all the hairs on the body are lost, it is called general baldness. The disease is related to immune disorders and sudden increase in stress. It is also related to deficiency of both qi and blood, liver and kidney, and blood stasis in the hair.

The cause of the disease is not yet known. Neuropsychiatric factors are considered to be an important factor. Many cases have neuropsychiatric trauma such as chronic anxiety, insomnia, apprehension, sadness, mental tension and emotional disturbance before the onset of the disease. Sometimes during the course of the disease, these psychiatric factors can make the condition rapidly worse. In recent years, studies have shown that the causes of baldness are related to the following factors.

1. Genetic allergy About 10% to 20% of cases have a family history. There are reports of monozygotic twins having baldness in the same area at the same time, and there are also reports of a family with baldness in all four generations, which is considered a genetic defective disease. From the accumulated clinical cases, we can see that people with genetic allergies are prone to baldness. 

2. Autoimmunity The rate of autoimmune diseases in patients with baldness is higher than that of the normal population. Dysfunction of the autoimmune system, or due to mental stimulation, emotional instability, anxiety, anger and fire, sadness, grief, sudden shock, or playing computer games at night, playing mahjong, or lack of sleep for a long time, etc. → dysfunction of the vegetative nerves → disorder of the vasomotor center → spasm and contraction of the local capillaries in the scalp → obstruction of blood supply to the hair papillae → early entry of the resting phase and loss of hair. 3. In Down syndrome, the incidence of baldness is increased, and it is often total or universal baldness. The presence of autoantibodies is significantly increased in these patients.

It is not certain that baldness is an autoimmune disease, but the fact that it can be accompanied by autoimmune diseases and is temporarily effective against corticosteroids suggests a preference for the autoimmune theory.

Pathological changes: lymphocytic infiltration around and below the hair follicle, partial invasion of the hair follicle wall, and degeneration of hair matrix cells. There may be new hair formation in the follicles of shed hairs. The newly grown hair lacks pigment. In advanced stages, the hair follicles, hair bulbs and their dermal papillae are shrunken and displaced upwards. The surrounding stroma is significantly reduced, the surrounding connective tissue is vascularized, and there is thrombosis of the blood vessels. The number of hair follicles also decreases with time, and cellular infiltration is not obvious at this time.

 
Clinical manifestations

Alopecia areata can occur at any age from infancy to old age, but is more common in middle-aged people, with no significant gender differences.
   
 
The disease is long discovered unintentionally or by others, and there are no conscious symptoms. In a few cases, the affected area may have a mild abnormal sensation at the beginning of the disease. It starts as one or several well-defined round or oval hair loss areas, and the straight play is about 1 to 2 cm larger. At the edge of the hair loss area, there are often some loose and easy to lose hair, some have been broken, and the hair at the proximal end tends to atrophy. If the hair is pulled out, the hair can be seen on the thick and thin like an exclamation mark (!) The lower part of the hair is also depigmented. This phenomenon is a sign of progression. The hair loss continues to increase and each piece expands and may fuse with each other to form an irregular shape. If it continues to progress, it can become completely bald. In severe cases, eyebrows, eyelashes, axillary hairs, pubic hairs and all over the body hair are also lost, which is called general baldness. The hair loss may also stop [2], at which time the hair loss area no longer expands, and the marginal hairs are also more solid and not easy to be pulled out, after a number of large, marginal hairs are also more solid and not easy to be pulled out, after a number of months, the hairs can gradually or rapidly grow out. There are also patients who first grow white fuzzy hair, and then gradually become thicker and darker, longer and become normal hair. The scalp of alopecia areata is normal, smooth and without inflammation, sometimes it looks thinner and slightly concave, which is due to the disappearance of hair and hair roots, not real scalp thinning. Baldness in children that occurs at the occipital hairline is called ophiasis.

Alopecia areata can occur in conjunction with the following conditions.

Differential diagnosis

It is not difficult to diagnose based on sudden onset, round or oval hair loss, and normal scalp in the area of hair loss. It must be differentiated from the following diseases.
 

1. alopecia areata Incomplete alopecia, most of the hairs are broken, the residual hair roots are not easily pulled out, and scales are attached. Mycobacteria can be easily detected in the broken hair. Prevalent in children.

2. Syphilitic baldness Although it is also patchy baldness, the hair does not have scar formation, but the edge is irregular and worm-like. The hair loss is also incomplete in the alopecia area and numerous, preferably on the posterior side. It is accompanied by other syphilis symptoms and a positive syphilis serology test.

3. Pseudo-baldness The affected scalp is atrophied, smooth and shiny, with no visible follicular openings and no hair loss at the edges of the patches that are thick at the top and thin at the bottom.
Complications

The appearance of pemphigus vulgaris is not isolated and is often accompanied by other diseases. (1) Nail lesions: The degree of lesions can be proportional to the degree of hair loss. (2) Genetic allergic diseases: often accompanied by allergic rhinitis, genetic allergic dermatitis, asthma and anaplasmosis. (3) Autoimmune diseases: including vitiligo, ulcerative colitis, pernicious anemia, systemic lupus erythematosus, rheumatoid arthritis, scleroderma, and myasthenia gravis. (4) Dowm syndrome: This disease is a genetic disease with chromosomal abnormalities, which suggests the genetic background of the disease. Treatment of baldness can be done with internal medication, external medication and dietary therapy.
 

Internal medicine 
There are a number of western medications that can be taken to treat baldness.

There is also the “6, 5, 4, 3, 2, 1 therapy”, which means 6 oral prednisone tablets (30 mg) in the first month, 5 tablets in the second month, and so on. These are safe and effective dosing regimens.
    (2) Other internal western drug therapies: Glycyrrhizin: The pharmacologically active unit is glycyrrhizin. Because it is similar to the molecular structure of corticosteroids, it has corticosteroid-like effects (i.e. anti-inflammatory, anti-metabolic and interferon-inducing effects), and rarely has the side effects of corticosteroids. Commonly used are: Glycine, Mennen tablets (compound glycine sweetener tablets), etc. , cystine tablets (300 mg daily) in 3 divided doses, and also glutamate tablets, B vitamins and vitamin E. Sedatives such as Benadryl, Valium, etc. are given to patients who are clearly psychiatric related.
    If individual cases are ineffective, internal treatment with 8 methocarbamol tablets such as long-wave ultraviolet radiation can be tried. In addition, some people have tried immunomodulators such as transfer factor and thymidine from the perspective of correcting possible cellular immune function defects, and received good results.    

External medicine
      Fresh ginger rubbing; wormwood 100 grams plus 400 grams of white wine, soaked for 1 week and then applied externally. This treatment for baldness is simple and fast; however, some people will have allergic reactions.
 
Western medicine topical treatment
 
① Minoxidil (minoxidi) topical treatment of hair loss mechanism.

Minoxidil is a vasodilator for the treatment of hypertension, but the mechanism of its treatment of hair loss is not mainly the effect of vasodilatation. According to experimental studies, the addition of minoxidil to in vitro cultures of hair follicles increases the growth time of cultured hair follicles. Classification of minoxidil solution

Minoxidil solution is divided into 2 types: 1 is the old formula of minoxidil hair growth water containing propylene glycol, 2 is the minoxidil series of solutions without propylene glycol

1) Minoxidil hair regrowth solution containing propylene glycol: the main ingredients contain alcohol, propylene glycol (30%), purified water, and 5% minoxidil (for men) or 2% minoxidil (for women).

The side effect of Minoxidil hair regrowth containing propylene glycol is that it contains propylene glycol as a solvent and therefore tends to cause increased dandruff and hair loss.

(2) In the overall use, more than 20% of patients are allergic to propylene glycol-containing minoxidil, resulting in increased dandruff, itchy skin, increased greasiness and, in severe cases, rapid hair loss. Statistically, Asian patients are more likely to be allergic to propylene glycol during use than patients in Western countries.
    ② Topical corticosteroid preparations are suitable for children and lighter patients. Topical corticosteroid creams, applied topically 2 times/d, have been shown to reduce perifollicular inflammation. However, the effect of topical corticosteroids alone in the treatment of baldness is not good, but is better when combined with minoxidil. 
 
 Closure therapy: Use the acetic acid or Debrisone A and 2% lidocaine suspension to inject the hair loss spot with point and fan-shaped closure, once a week, usually within 1~2 weeks to control the condition and gradually grow new hair.
 

Alopecia Areata Diet Therapy

Diet therapy for alopecia areata is to promote hair growth and prevent or improve hair loss by supplementing the diet with foods that are beneficial to hair growth and health.

Attention is paid to the selection of foods beneficial to hair growth in the diet.
