① Issued by the Ministry of Health in February 2011 ② Glucocorticoids have a wide and complex range of effects, and vary with dose; glucocorticoids secreted in physiological state – mainly affect the process of substance metabolism. Glucocorticoids applied in supraphysiological doses – anti-inflammatory, anti-immune, anti-toxic, anti-shock and other pharmacological effects. ③Cortisone 25mg=hydrocortisone 20mg=prednisone 5mg=methylprednisolone 4mg=dexamethasone 0.75mg, the same anti-inflammatory effect when taking equivalent doses of glucocorticoids; ④The correct choice according to different diseases and the characteristics of various glucocorticoids; ⑤You cannot use glucocorticoids purely for the purpose of reducing fever and relieving pain. Glucocorticoids have pharmacological effects of suppressing autoimmunity, but they are not suitable for the treatment of all autoimmune diseases such as chronic lymphocytic infiltrative thyroiditis (Hashimoto’s disease), type 1 diabetes, and common psoriasis. (6) Clinical application of glucocorticoids in endocrine diseases Used in the replacement therapy of primary and secondary hyperalgesia and congenital adrenocortical hyperplasia; rescue of adrenal crisis, pituitary crisis, thyroid crisis and other emergencies; severe subacute thyroiditis, Graves’ ophthalmopathy, etc. (7) High and low dose dexamethasone suppression test can determine the status of adrenal cortical secretion. I. Small dose dexamethasone suppression test (LDDST): Standard 48-hour dexamethasone suppression test: 0.5 mg of dexamethasone is given orally every 6 hours for 2 days, and blood cortisol levels are measured 48 hours after the first administration of dexamethasone. First-line screening Overnight dexamethasone suppression test: A single oral dose of dexamethasone 0.5-2mg (commonly 1mg) at 23:00 midnight, and blood cortisol level is measured at 8:00 am on the second day. Mostly used in outpatient clinic Domestic dexamethasone single dose is 0.75mg, in practice in the overnight method 1.125mg (1½ tablets), in the classical method given 0.75mg every 8 hours for two days. II. High-dose dexamethasone suppression test (HDDST): the most classical method to identify Cushing’s disease from adrenal adenoma Standard HDDST: 2 mg of oral dexamethasone every 6 hours for 2 days in 48 hours, in practice 2½-3 tablets-2½-3 tablets can be given every 6 hours (total daily dose of 8.25 mg) for two days.
