Statins are hydroxymethylglutaryl coenzyme A (HMG-CoA) reductase inhibitors, which can effectively lower total serum cholesterol and low-density lipoprotein. At present, the main clinical applications in China are simvastatin, pravastatin, fluvastatin, atorvastatin and rasulvastatin. When taking statins, patients need to pay attention to the type of drug, time of administration, diet, etc., and also pay close attention to the body’s reaction. 1. Type of drug: There are many types of statins, and the common oral doses of different types of drugs are different, and the specifications of the same type of statin from different manufacturers also vary. Statins are not affected by food, and meals have little effect on the functioning of the drug, and statins generally have no serious gastrointestinal adverse reactions. Most statins are best taken at night before bedtime because the peak of cholesterol synthesis is around 12:00 midnight, and bedtime dosing allows the drug to reach its peak at midnight, thus better lowering cholesterol. Atorvastatin and Rosuvastatin can be taken at any time of the day because of their long half-life. If you miss the dosing time, you should make up for it immediately when you remember. If you are close to the next dosing time, you do not need to make up for it, and do not use double dose at one time privately. 3. Pay attention to elevated liver enzymes: Generally speaking, patients taking statins are safe and effective, but a small number of patients will have elevated liver transaminases after taking statins. For patients with mild to moderate abnormalities in transaminases, they should not be treated with statins if they have active hepatitis and other serious liver diseases. If a patient has NAFLD in combination, application of statin is not only safe and effective, but also improves liver function. Therefore, statin should be used actively and adequately in patients with indications for statin without evidence of organic liver damage such as hepatomegaly, jaundice, or elevated direct bilirubin. After taking statin, as long as the patient has no progressive increase in transaminases (≥3 times the upper limit of normal), the drug should be continued; 4. Pay attention to myalgia reaction: muscle pain is one of the common adverse reactions of statins, which can be divided into myositis and myalgia according to whether the creatine kinase (CK) value is normal or not, and if CK is greatly increased, it may be rhabdomyolysis, which can even be life-threatening in serious cases. Patients in the application of statins, as long as the symptoms of suspected muscle discomfort, should immediately stop the drug, timely medical care, so as not to delay the disease; 5, pay attention to the combination of drugs: the combination of drugs is statins, due to liver damage, muscle toxicity is an important risk factor. If you take azithromycin, amiodarone, roxithromycin, fenofibrate and other hepatic enzyme inhibitors at the same time, it can make the blood concentration of statins rise, increasing the probability of adverse reactions. Some patients have a substantial increase in triglycerides, and the need to take fenofibrate and statin at the same time, can take fenofibrate in the morning and statins at night before bedtime, which can reduce the incidence of adverse reactions; 6, pay attention to diet: do not overconsume grapefruit grapefruit products during the taking of statins, which can slow down the metabolism of statins and easily induce adverse reactions. Patients should also pay attention to improve lifestyle, low salt, low fat, balanced diet, reduce cholesterol intake, in addition to also strongly advocate moderate exercise.
