Lumbar disc herniation is a common clinical disease in orthopedics and traumatology, with a prevalence rate of 5.1% for men and 3.7% for women in the population. It is one of the characteristic diseases treated by TCM, with significant clinical efficacy, easy acceptance by patients, and no obvious side effects and adverse reactions. I will use the characteristics of our school of Chinese medicine in the treatment of lumbar disc herniation in recent years to learn and promote each other, and continuously improve the diagnosis and treatment for the benefit of the majority of patients. We believe that lumbar pain is related to internal and external factors such as wind and cold, blood stasis, liver and kidney. As the “Treatise on the Origin of Diseases? The “Stroke lumbar pain” said that “the injured person, the kidney deficiency, and the kidney is the main waist and foot, and its meridian through the kidney contact ridge, the wind evil take advantage of the deficiency, the stroke into the kidney meridian, so the stroke is lumbar pain.” . Also “the source of diseases? Waist and foot pain,” said “kidney deficiency, by the wind evil, labor injury is kidney deficiency, deficiency is affected by the wind and cold, wind and cold and the real gas fight so waist and foot pain.” . Based on the principle of treating the symptoms in an urgent manner and treating the root cause in a slow manner, we believe that the “pain” of lumbar disc herniation is the symptom, and the key to treatment is to remove wind and cold, invigorate blood and relieve pain, so as to relieve and eliminate pain. Through long-term clinical practice, it is believed that most of the patients with lumbar herniation have the following characteristics: First, they feel wind and cold, and the cold evil stays in the meridians, resulting in a loss of harmony between qi and blood, resulting in symptoms such as lumbar pain and numbness; second, they suffer from long-term cumulative strain, such as prolonged standing, prolonged sitting, heavy lifting and other labor, which leads to liver and kidney deficiency over time and develops under the causative factors of trauma or cold; third, they have an obvious clinical history of sprains and injuries. Due to sudden sprain, blood stasis blocked the meridians and channels, and if the blood does not pass through, the pain will occur. According to the clinical manifestations and physical differences of the patients and the characteristics of the onset of the disease, we put forward the academic thought of dividing the treatment into three stages and identifying the types of symptoms. According to the clinical manifestations and course of the disease, it is divided into acute attack period, remission period and recovery period. (1) Acute phase: According to the different causes of the disease, the evidence is divided into wind-cold type and blood stasis type. (1) Wind-cold type: wind-cold is induced by the injury or strain, which induces lumbago. The treatment is to relieve wind and dispel cold, invigorate blood and relieve pain. Shi’s wound ointment can be used externally. Or apply hot compresses with Shu Shu Shu Blood. ②Blood stasis type: due to acute sprain, tendons and veins are damaged, Qi stagnation and stasis, and pain if not pass. The treatment is to remove blood stasis and clear the knot, and the formula is used to activate blood circulation and relieve pain. The formula uses angelica, red peony, Chuanxiong, safflower, peach kernel, ground turtle worm, Liu Zhongyu, Lodder, frankincense, myrrh, panax ginseng, cow’s knee, Xuanhu, white peony, Di Long, stretch grass, Citrus aurantium, Chen Pi, etc. plus and minus. Externally, use Shi’s new wound ointment. Or apply hot compresses with Shu Tendon and Blood Dispersion. (2) Relieving period: Pain in the lower back and legs has been relieved compared with the acute period, but the vague pain still exists, at this time the qi and blood have not yet been harmonized, although the wind and cold have not been removed. The treatment is to relieve the wind and invigorate the blood, harmonize the camp and open the ligaments, and the formula is Shu Feng and Invigorate Blood Soup. The formula is based on the formula of Shu Feng Invigorating Blood Tang. The medicine is reduced with the addition of Fang Feng, Dou Wu, Gentiana Macrophylla, Angelica Sinensis, Red Peony, Chuan Xiong, Wei Ling Xian, Wu Gan Pi, Niu Knee, Fang Qi, Sang Sang Sang, Sequoia, Du Zhong and Chen Pi. Shi’s wound ointment can be used externally. Or apply hot compresses to the wound. (3) Recovery period: The pain symptoms basically disappear, but the back and legs feel sore and weak, can not stand for a long time, mostly due to liver and kidney deficiency, lack of qi and blood, clinically can be divided into Yang deficiency and Yin deficiency two types. For Yang deficiency, the treatment is to warm the kidney and Yang, nourish the blood and strengthen the waist, and the formula is to nourish the kidney and strengthen the waist; for Yin deficiency, the treatment is to nourish the Yin and nourish the blood and strengthen the waist, and the formula is to nourish the Yin and strengthen the waist. The formula uses Shu Di, Yam, Cornu Cervi Pantotrichum, Fructus Lycii, Radix Angelicae Sinensis, Rhizoma Chuanxiong, Radix Paeoniae Alba, Rhizoma Cistanches, Radix Gentianae Macrophyllae, Radix et Rhizoma Millenium, Radix et Rhizoma Dogwood, Radix Achyranthes Bidentatae, Radix Eucommiae. Or apply hot compresses to soothe the tendons and activate the blood. On the basis of the above dialectical treatment, it is believed that “cold and dampness with stagnation, Qi and blood blockage, and pain if it does not pass” is the important cause of this disease. According to this theory, the Chinese medicinal preparation Shu Feng Shu Waist Granules has the clinical effect of dispersing cold, activating blood circulation and relieving pain, and the animal test proved that Shu Feng Shu Waist Granules has obvious anti-inflammatory effect, and has certain effects of relieving pain, reducing capillary permeability and improving blood rheology [3]. In clinical practice, Shufeng Shuwa granules can reduce the swelling, inflammation and nerve root edema of soft tissues, achieve the relief of nerve root compression, reduce pain and promote the repair of soft tissue lesions, while having certain efficacy on lumbar disc herniation. The clinical efficacy study showed that the excellent rate of Shu Feng Shu Waist Granule reached 88.2%, which was significantly different from the control group. This technique is originated from the late Mr. Lu Wen, a famous Tui-Na practitioner of our hospital. As one of the schools of massage in Shanghai, Lu Wen’s technique is unique in osteopathic massage, with its unique method and remarkable curative effect, so it is widely spread among the people. Together with Academician Chen Zhongwei, the predecessors of our department have conducted in-depth research on it using modern science and technology and means, revealing part of the mechanism that the manipulation can improve and adjust the relationship between the herniation and the contents of the spinal canal and nerve root canal, so that the nerve root can be pulled, thus improving the condition of lumbar intervertebral disc herniation and providing a scientific theoretical basis for clinical application. The specific operation method is to use a variety of treatment methods in three positions: (1) sitting position: ① thumb push and knead method: the patient takes a sitting position, the operator sits low behind the patient, push and knead around the pressure point with the thumb push and knead method for about 2 minutes, after the pain is slightly relieved, the patient leans back against the operator’s head, push and knead with the thumb push and knead method from light to heavy to stimulate the pressure point for about 3 minutes, then from the spinous process to the lumbar ribs on both sides, use the Thumb wipe method 10 times to release muscle spasm and relieve pain. The patient takes a sitting position, crosses his arms and holds his shoulder joints, and the assistant clamps the patient’s legs on the inside of both legs, and presses the palms of both hands against the anterior part of the iliac crest on both sides of the patient to fix the pelvis and not to make it turn. The operator stands on the right posterior side of the patient, the left hand pulls the right wrist of the patient, the right hand pushes the right posterior shoulder of the patient, makes the patient tilt back 40°~50°, the waist is relaxed as much as possible, generally the upper part of the trunk can be turned 70°~80°, so that the trunk muscles are in a rather tense state, then, the operator suddenly increases the twisting angle by force about 10°~20°, at this time the small joints of the waist can produce a crisp “clucking” sound, and then the same method to the opposite direction once. (2) Prone position: ① Lift the leg and press the waist method: the patient lies prone on a hard bed, the operator stands on the affected side of the patient, presses the pressure point of the herniated nucleus pulposus gap with the palm of one hand, or the lateral protrusion center laterally, and lifts the lower end of the affected thigh with the other hand upward, or the lower end of both thighs, so that the waist is overextended until the muscles in front of the abdomen and hip are very tense and the patient feels swollen in the waist. At this point, the operator also needs to slightly and suddenly increase the upward lifting and wrenching techniques. ② stepping method: the patient takes the prone position, the chest and abdomen are each padded with soft cushions, and the two assistants each hold the axilla and ankle for antagonistic traction. One foot of the operator stands on the patient’s sacrum, and the heel of the other foot is placed on the spinous process or paravertebral pressure point in the center of the lateral eminence, and the two assistants tug and the second assistant lifts the thigh slightly upward to step on the middle and lower front. The force is light to heavy, and the point of force should be on the spinous process. It is contraindicated in people with poor physical condition or osteoporosis. (3) supine position: ① foot dorsiflexion method: the operator uses the palm of the hand on the same side as the patient’s affected limb to hold up the patient’s heel posteriorly, the palm of the forearm against the plantar anterior, the other hand presses the affected knee to straighten the affected foot to force dorsiflexion, straight leg elevation under dorsiflexion to the maximum tolerated by the patient, and then suddenly slightly increase the dorsiflexion technique. ② hip flexion and knee pulling method: the operator’s hands pressed the knee and lower leg, respectively, to the patient’s chest for downward pressure, and then quickly force the affected limb straight, repeated several times. This technique has the effect of pulling the sciatic nerve, and has the effect of moving and loosening the hip and knee joints. The above representative methods can be applied selectively according to the patient’s condition and physical condition at the time of clinical evidence, or combined with the flicking method, tiger mouth pushing and kneading method, acupressure method, lifting and kneading method, tilting and wrenching over extension method, etc. In addition, we believe that the key to the use of Chinese medicine in the treatment of lumbar disc herniation is correct dialectical evidence, and only on the basis of accurate dialectical evidence is the targeted use of the mastered Chinese medicine treatment methods.
