What’s this soft mass inside the scrotum?

A soft mass inside the scrotum may be a normal epididymal structure or it may be the pathologic structure of testicular syringomyelia, an epididymal cyst. 1. Epididymis: It is a structure composed of a mass of twisted and tiny tubes connecting the testis and the vas deferens, and its main function is to temporarily store semen and nourish spermatozoa, and to promote the further maturation of spermatozoa. The normal structure is close to the upper and posterior edge of the testis. When palpated, the normal epididymis is softer and more resilient. 2. Testicular syringomyelia: It refers to the cystic changes that gradually form when too much fluid accumulates in the testicular sheath. The main causes are delayed development of syringomyelia with late absorption, inflammation of the syringomyelia, or secondary to orchitis, epididymitis, hernia surgery and scrotal surgery. 3. Epididymal cyst: It is a cystic mass that occurs in the epididymis, mostly due to semen stasis.
