In the early stage, stroke patients spend most of their time in bed, so the type of position adopted is very important.
Good limb position placement is the most basic treatment for stroke patients in the early stage, which can play a good role in inhibiting spasticity patterns (upper limb flexor spasm and lower limb extensor spasm), preventing shoulder subluxation, and inducing early dissociative movements. It is generally recommended to change the patient’s position once every 2 hours. When the patient can turn over in bed or move actively, the interval can be changed appropriately.
Good limb position placement
Supine position.
1. the bed must be as flat as possible.
2, head position should be fixed on the pillow, not flexible and able to move.
3.Bilateral shoulder joint: fixed on the pillow.
4. Upper limbs on the hemiplegic side: fixed on the pillow and trunk at an angle of 90 degrees; elbow, wrist and finger joints should be straightened as much as possible.
5.Hip on the hemiplegic side: fixed on the pillow; the upper limb on the hemiplegic side is also placed on the same pillow.
Reclining on the affected side.
1. The bed must be as flat as possible.
2. The head position should be fixed.
3. the trunk is slightly backward, and a pillow is placed behind and on the head to fix it.
4. Shoulder joint on the hemiplegic side: forward flat extension and internal rotation.
5. Upper limb on the hemiplegic side: 90 degrees from the trunk, place a small table by the bed and put the hand completely on it; straighten the elbow joint as much as possible with the palm up.
6. Lower extremity on the hemiplegic side: knee slightly bent, hip straightened.
7. upper limb on the healthy side: placed on the body or on a pillow.
8.Healthy side lower limb: keep stepping position, put on pillow; knee and ankle slightly flexed.
Healthy side lying position.
1, the bed must be as flat as possible.
2, the head position should be fixed, and the trunk in a straight line.
3.Torso slightly tilted forward.
4.Shoulder joint on the hemiplegic side: forward flat extension.
5. Upper limb on the hemiplegic side: put it on the pillow and make an angle of 100 degrees with the trunk.
6.Lower limb on the hemiplegic side: knee and hip slightly bent; leg and foot on the pillow.
7. Upper limb on the healthy side: how comfortable the patient sleeps.
8.Lower limb on the healthy side: knee and hip straightened.
Bed sitting position.
1, the bed as flat as possible, the patient’s lower back put pillows.
2, head: do not fix, able to move freely.
3.Torso: straighten.
4.Hips: 90 degrees of flexion, with weight evenly distributed on both sides of the hips.
5, upper limbs: placed on an adjustable table with a pillow on top.
Sitting in a chair or wheelchair.
1, a pillow placed on the lower back.
2, the patient’s hands are stretched forward with the elbows on the table to transfer the correct posture of the hands.
3, feet flat on the floor, or on a flat bench.
