365medical.com: Hello, Professor Zhang! Thank you for accepting our interview in your busy schedule. The First Annual National Academic Conference of the Vascular Disease Committee of the Chinese Society of Microcirculation and the 2014 Chaoyang Vascular Medicine Forum will be held in Beijing from November 7 to 9, 2014. And what are the highlights and features of the forum? Prof. Zhang Wangde: The Chinese Society of Microcirculation was established on August 30, 1993, and its first chairman was Professor Xiu Ruijuan, a famous medical scientist and the creator of “Xiu’s theory”. Microcirculation is an interdisciplinary discipline that relies on modern high-tech means and is closely related to basic research and clinical practice in various medical fields. It studies the interactions and functional regulation of blood, blood flow, blood vessels and surrounding tissues under physiological and pathophysiological conditions from the organ-tissue and cell-molecular levels in the microvascular sphere. This conference will formally announce the establishment of the Vascular Diseases Committee of the Chinese Society of Microcirculation, and the first annual academic meeting will be held to better expand the functions of the Society, optimize the academic ecological environment, highlight the advantages of cross-fertilization between medical and scientific disciplines and facilitate the translation of basic and clinical medicine, and promote academic exchange and cooperation. The conference will invite famous scholars from home and abroad, such as academician Zhan Qimin, academician Wang Zhonghao, Guan Heng and Wu Qinghua, as speakers to introduce the cutting-edge research in the field of international microcirculation and vascular diseases. The conference will be attended by experts in multiple fields such as vascular surgery, cardiac surgery, hematology, cardiology, radiation intervention and general surgery, etc. The conference will focus on highlighting the advantages of multidisciplinary professional crossover and promoting academic exchanges. In addition, the conference has a 2014 vascular medicine young physician forum, so that more young doctors can show their style, get exercise and performance, which is conducive to the discovery of talent, cultivate talent, and also show the vitality of the development of vascular medicine. 365medicine.com: With the change of diet structure and the progress of population aging, the incidence of atherosclerotic diseases is increasing year by year in China, which has become one of the important diseases endangering human health. Can you tell us your opinion on the intracavitary treatment of atherosclerotic diseases? Prof. Zhang Wangde: The danger of cardiovascular and cerebrovascular diseases to human body has been generally recognized, but many people still lack the knowledge of peripheral vascular diseases, in fact, atherosclerotic diseases are systemic diseases, 27.8% of patients with peripheral vascular diseases also suffer from cardiovascular or cerebrovascular diseases, but some research reports point out that the diagnosis rate of peripheral vascular diseases in China is less than 2%, we should raise the awareness of peripheral vascular diseases, achieve early diagnosis and treatment. We need to raise the awareness of peripheral vascular diseases, achieve early diagnosis and early treatment. Interventional endoluminal therapy is currently the preferred and leading treatment method for this type of disease. Such methods are on the one hand advances in material science and on the other hand innovations in technology, which should make full use of the new achievements in human development for the benefit of mankind and promote the level of health. Endoluminal treatment has the advantages of minimally invasive and fast recovery, but it is currently expensive and not all cases are suitable for one fixed modality. We call for breaking down disciplinary limitations and achieving multidisciplinary cooperation to choose the right treatment for different patients. Hybridization is a way to combine traditional surgery with endoluminal interventional technology. On the one hand, we should further develop endoluminal minimally invasive surgery, and on the other hand, traditional surgery should not be abandoned. The treatment of atherosclerotic diseases should also emphasize comprehensive treatment, not to superstitiously believe in a certain method to solve all problems, and not to think that the disease is cured after the surgical technology intervention and “once and for all”. It is necessary to give up the bad lifestyle, stop smoking, limit alcohol, pay attention to healthy diet, regular life and moderate exercise. We can’t ignore the medical medication, control “three highs”, antiplatelet, lipid, sugar, blood pressure treatment, etc. 365medicine.com: At present, lower extremity venous disease is attracting more and more attention, its incidence is high and involves a wide range of people, which has caused a lot of pain to people’s life for a long time. We know that you have rich clinical experience in vascular surgery, can you share with us your valuable experience and insights in the treatment of lower limb venous diseases? Prof. Zhang Wangde: Lower limb vein diseases are mainly varicose veins and lower limb vein thrombosis. As people’s pursuit of beauty requires not only the cure of lower limb varicose veins, but also the aesthetic and small trauma, we use minimally invasive small incision combined with TRIVEX rotary treatment or combined with laser treatment to cure a large number of patients with lower limb varicose veins, so that the excision is more complete, does not affect the aesthetics, and is not easy to recur. Combined with the application of venous active drugs such as abortifacient and Aviron, we promote early recovery after surgery. Lower extremity deep vein thrombosis leads to pulmonary embolism. Our respiratory department has disciplinary advantages in the treatment of pulmonary embolism, and we currently use a comprehensive treatment based on anticoagulation, emphasizing early, timely, adequate and individualized anticoagulation. According to the new theory, we do not particularly restrict the patient’s activity, combine with elastic support therapy to reduce the spread of thrombus and prevent post-thrombotic syndrome. For the new oral anticoagulants such as Bactrim, we strictly follow the dosing norms and use them rationally to reduce the risk of thrombotic recurrence and pulmonary embolism. 365medical.com: We understand that this year is the 15th anniversary of the establishment of the Department of Vascular Surgery of Chaoyang Hospital. So far, the Department of Vascular Surgery of Chaoyang Hospital has developed into a department with a high reputation in China and the treatment technology is at the advanced level in China. Can you tell us how you have achieved your current achievements and your future development plans for the department? Prof. Zhang Wangde: The Department of Vascular Surgery in Beijing Chaoyang Hospital was founded by Professor Yuan Chao, one of the oldest vascular surgeons in China, and has gone through 15 years of history. In 2003, we jointly held the National Symposium on Carotid Vascular Diseases with the Department of Vascular Surgery of Beijing China-Japan Friendship Hospital. We have done a lot of work on the endoluminal treatment of lower limb artery disease, aortic disease and carotid artery disease, which has been recognized by our colleagues. This is an early academic exchange meeting of intracavitary vascular surgery in China. Professor Yuan Chao has made outstanding contributions to the development of vascular surgery in Chaoyang Hospital. In recent years, due to well-known reasons, the development of vascular surgery in Chaoyang Hospital has gone through a period of twists and turns. In 2013, the direction of development of vascular surgery in Chaoyang Hospital is “consolidate the foundation, keep an eye on the frontier, highlight the minimally invasive, adhere to the characteristics, and break out a new path”. On the one hand, we will continue to consolidate and develop “TRIVEX minimally invasive rotational treatment for varicose veins”, and on the other hand, we will strengthen the development of endoluminal interventional treatment and hybrid surgery for various vascular diseases, and we can carry out various interventional procedures or hybrid surgery in the operating room. On the one hand, we have to keep an eye on the direction of international vascular surgery development, we strengthen the learning of new technologies and increase the training of talents in the treatment of aortic diseases, so that the medical quality and efficiency can be significantly improved. We also need to highlight the characteristics of the department, the treatment of diabetic foot, treatment of deep vein thrombosis of lower limbs, carotid artery stenosis treatment features to further highlight the characteristics of medical treatment to attract patients to seek medical treatment, now a considerable number of foreign patients come to our hospital to seek medical treatment. The development of our vascular surgery department is starting to enter the fast lane. In the future, we will continue to uphold the spirit of integrity, kindness and diligence, continue to work hard, strive to forge ahead, actively explore and develop our vascular surgery department better! ?
