What is a herniated lumbar disk?

I have been practicing medicine for 20 years, and I often encounter patients with lumbar disc herniation in my outpatient clinic (including specialists and experts), and he/she often asks me. Lumbar disc herniation is what is going on? I told him (her): everyone has lumbar intervertebral discs, the heaviest load on the human body is lumbar 4/5, lumbar 5/sacral 1 intervertebral discs, with age or lumbar strenuous/excessive exercise caused by the disc nucleus pulposus, fibrous annulus, resulting in the fibrous annulus rupture, the nucleus pulposus protrudes, irritation or compression of the nerve root (mainly the sciatic nerve) to produce a series of symptoms and signs. The main manifestation is lumbar and leg pain, radiating from the waist and buttocks along the posterior and lateral thighs to the calves and dorsum of the feet. The pain worsens when coughing, sneezing and defecating and is relieved by rest. In severe cases, the cauda equina nerve can be compressed, causing urinary and bowel problems.
