  Diabetes is a strange disease, when people have not yet understood what is diabetes when they have unknowingly suffered; when people are trying to eradicate diabetes, but found that the next generation of blood sugar also began to rise; when people just happy to live a rich life, do not have to work hard all day long, but found that diabetes has come unexpectedly. This is a disease that loves to get on people’s nerves, a disease that allows people to “suffer” and not enjoy the benefits, a real “strange disease”.  In the process of diagnosing and treating diabetes, I also feel that there are some strange things about diabetes, which are listed below, such as the diagnosis is sometimes difficult to determine. Blood glucose often fluctuates, and after a period of diet control or increased exercise, especially after a significant weight loss (e.g., after a 3-5 kg reduction), blood glucose often drops significantly, and at this time, when blood glucose is rechecked, it is not even enough for the diagnosis of diabetes. Some of my friends often ask me this question when they visit me, that is, after the control of blood sugar down, is it still diabetes? At this point, I will make a joke: “You changed your clothes today, are you still you?” Once we are diagnosed with diabetes, we actually do not need to do the confirmatory test repeatedly, but should seriously face the fact of diabetes, take the necessary lifestyle adjustments (diet, exercise, psychology, sleep, etc.), take medication as prescribed by the doctor if necessary and monitor it on time, and adhere to it for a long time. Even after control, without medication blood sugar has returned to normal, but still can not easily “remove the cap”, the purpose is only one, that is, to promote the patient long-term attention. Often, when the psychological relaxation is the time when the rise of diabetes blood glucose is back, not now, but also within a few months. In this sense, diabetes is a test of human nature, it forces you to remain diligent, remain self-reflective, not humble and not overbearing (not too pessimistic, not too optimistic) You say that this disease is strange?  Diabetes is “strange” not only because of the fluctuations in blood sugar, so that the diagnosis often varies from one day to another, so much so that some people have been ill for several years and are still undergoing confirmatory tests, especially those who have only mildly increased blood sugar. Fluctuations in blood glucose often make their diagnosis sometimes valid and sometimes difficult to determine. So it is recommended that those who are very close to the diagnosis or once the data reaches the diagnostic criteria be aware of the existence of diabetes in time, whether it is diagnosed or there is still a slight gap, to pay enough attention to avoid delaying the disease.  Another strange thing about diabetes is that it is often related to weight. Some people think that you need to be fat or middle-aged or elderly to have diabetes. In fact, it is not uncommon to find people with diabetes who have just grown a “small belly” or are in their twenties. Asian populations often have a high body fat content [BMI = weight (kg) / height2 (m2) for the same BMI]. Some people often develop diabetes without waiting to grow into a big fat person. And some people who are overweight or obese from a young age have high blood sugar before they reach middle age. These are due to excess body fat, especially excess visceral (torso) fat, which prevents the body from using glucose, thus causing them to have high blood sugar after eating. Recognizing this, it is often possible for people who are overweight or obese to bring down already elevated blood sugar if they can successfully lose weight and maintain it. If the ideal weight is maintained over time, diabetes can often be “cured”. However, it is important to note that both weight loss and maintenance are important. Even if you cannot lose a lot of weight at once, your blood sugar will become more normal as you lose weight gradually. So the weight loss process is also the process of the body recovering from diabetes.  Here is a short story to illustrate the importance of fat loss for diabetes recovery. A few months ago I saw a 27 year old guy who had just been married for a year, was 5’5″ and weighed 95 kg. He said he was 95 kg when he was 18 years old and his blood pressure and blood sugar were already elevated at that time. His family sent him to the army in order to make him lose weight. A year of recruit training required a brisk march of 35 to 50 kilometers a day. This way he lost 25 kilograms a year, and his blood sugar and blood pressure were completely normal by then. This is the second oddity of diabetes, closely related to weight, to treat or recover, without losing weight seems to be impossible. Weight, or to be precise, body fat, for every kilogram lost, the body’s glucolipid metabolism will become more normal.  There are two meanings to say here: one is for people whose blood sugar is already high and has not reached the diagnostic standard for a while; or who have been diagnosed at one time and have normalized their blood sugar after treatment. Never relax the diet control and exercise exercise, to realize that diabetes should be said to be a kind of body, in certain conditions to show the state of high blood sugar, once the conditions relaxed, it will make high blood sugar appear. Therefore the diagnostic criteria are only for discovering patients with, once diagnosed, there is no need to repeatedly do the confirmatory test, you should immediately follow the lifestyle requirements of diabetes for self-management, of course moderate and long-term, not overnight. In addition, once your doctor recommends medication, you should take it promptly as prescribed. It is understandable to be paranoid about the disease, but not paranoid. Another thing is that once overweight or obese people find that their blood sugar is increased, they should try to make weight loss. But weight loss is like sailing against the current, need to continue to work, once the weight has rebounded, do not give up on yourself. It is important to know that as long as you adhere to certain dietary principles and active exercise, even one day will have a day of gains. Each day of persistence, blood sugar will be more ideal day, in the long run, is not more healthy than healthy people? What does it matter if you don’t lose a lot of weight or if you don’t lose any?
