The diagnostic criteria for anxiety disorders in the Chinese Classification and Diagnostic Criteria for Mental Disorders, 3rd Edition (CCMD-3) are as follows.
Diagnostic criteria for anxiety disorders: It is a neurological disorder in which anxiety is the predominant emotion. It is mainly divided into two types: panic disorder and generalized anxiety. The anxiety symptoms of anxiety disorders are primary. Any anxiety secondary to somatic diseases such as hypertension, coronary artery disease and hyperthyroidism should be diagnosed as anxiety syndrome. Other psychopathological states such as hallucinations, delusions, obsessive-compulsive disorder, hypochondria, depression, phobias and other concomitant anxiety should not be diagnosed as anxiety disorders. Diagnostic criteria for panic disorder: It is a neurological disorder with recurrent panic attacks as the main primary symptom. Such attacks are not limited to any specific context and are unpredictable. Panic attacks as secondary symptoms are seen in many different psychiatric disorders, such as phobic neurosis and depression, and should be differentiated from certain physical disorders, such as epilepsy, heart attack, endocrine disorders, etc.
Symptom criteria] (1) meet the diagnostic criteria of neurosis; (2) panic attacks need to meet the following four items: (1) no obvious trigger for the attack, no relevant specific context, the attack is unpredictable; (2) in the interictable period, there are no obvious symptoms except the fear of another attack; (3) the attack shows strong fear, anxiety, and obvious autonomic symptoms, and often have depersonalization, reality dissolution, near-death fear, or loss of control (4) The seizure starts suddenly, reaches its peak quickly, and the patient is clearly conscious during the seizure and can recall it afterwards. Severity Criteria】The patient feels pain because it is unbearable and cannot be relieved. 【Criteria for disease duration】 At least 3 panic attacks within 1 month, or anxiety secondary to the fear of another attack lasting for 1 month after the first attack. [Exclusion criteria] (1) Exclude other mental disorders, such as phobias, depression, or somatoform disorders secondary to panic attacks.
Diagnostic criteria for generalized anxiety: It refers to an anxiety disorder that is dominated by anxiety with a lack of clear objects and specific contents, and tension and anxiety with significant vegetative symptoms, muscle tension, and motor restlessness. The patient feels distressed because it is difficult to bear and cannot be relieved. [Symptom Criteria] (1) Meet the diagnostic criteria of neurosis; (2) Have predominantly persistent primary anxiety symptoms and meet two of the following: (1) Frequent or persistent fear or nervousness without a clear object or fixed content; (2) With autonomic symptoms or motor restlessness. [Severity criteria] Social function is impaired, and the patient feels distressed because it is unbearable and cannot be relieved. 【Criteria for disease duration】 Meet the criteria of symptoms for at least 6 months. [Exclusion criteria] (1) Exclude secondary anxiety from somatic diseases such as hyperthyroidism, hypertension, coronary heart disease, etc.; (2) Exclude overdose of excitatory drugs, hypnotic sedative drugs, or withdrawal reactions to anxiolytics, anxiety associated with obsessive-compulsive disorder, phobia, hypochondria, neurasthenia, mania, depression, or schizophrenia.
