In severe cases of bunion, conservative treatment is no longer indicated and surgical treatment is the only option for correction. First, the medial joint capsule needs to be incised, the medial bursa needs to be cut off, the medial bunion bursa needs to be exposed, and the bunion bursa needs to be completely bitten off using a bite forceps, and then the medial adductor tendon needs to be cut and the medial joint capsule needs to be tightly sutured to correct the bunion. Alternatively, if the above measures do not result in satisfactory bunion correction, wedge osteotomy of the first metatarsal can be considered. Wedge osteotomy can better correct particularly severe bunions, but requires internal fixation with kerf pins or miniature metacarpal plates.
