Whether or not chronic lymphatic leukemia in the elderly is life-long and requires no treatment depends on whether or not indications for treatment have arisen throughout the course of the patient’s disease. Chronic lymphocytic leukemia, whether old or young, does not require treatment once diagnosed. Patients without treatment indications are watchful waiting, and only those with treatment indications need to initiate treatment. About one-third of patients with chronic lymphocytic leukemia will never need treatment. One-third of patients with chronic lymphocytic leukemia need to start treatment as soon as they are diagnosed. Another third of patients with chronic lymphocytic leukemia are initiated only when treatment indications arise during the watchful waiting process. Specific therapeutic drugs include ibrutinib, nitrogen mustard phenylbutyrate, and cyclophosphamide, and the use of the drugs needs to be under the supervision of a physician.
