Most anal fistulas are caused by perirectal abscesses, which can form if the abscess breaks down on its own or if the abscess is incised and drains into an external opening. As external wounds grow and heal more quickly, fistulas often pseudo-heal, leading to recurrent abscess ruptures and the formation of multiple external openings, making simple anal fistulas complex.
