Erythema and pain in the foot is an acute inflammation of the skin and its reticular lymphatic vessels. It is more common in the lower extremities and face. The clinical manifestations are rapid onset, with a localized, well-defined, flaky red rash that is bright red and slightly elevated, and fades when pressed. The skin surface is tense and hot, spreading rapidly in all directions, with burning pain. It is accompanied by high fever, chills and headache. Although the name of the disease is “poison”, it is not caused by a viral infection, but an acute purulent dermal inflammation caused by a bacterial infection. The pathogenic bacteria is group A type B hemolytic streptococcus, mostly by the skin or mucous membrane wound and invasion, but also by bloodstream infection. The etiology of dermatophytosis is group A type B hemolytic streptococcus, occasionally caused by type C or type c streptococcus. Most of them are invaded by the breakage of skin or mucous membrane, and can also be infected by bloodstream. Other factors such as malnutrition, excessive alcoholism, gammaglobulin deficiency and nephrogenic edema can all contribute to the development of dermatitis. The clinical manifestations of dermatophytosis are: general malaise, chills, nausea and other symptoms before the onset of the disease, followed by the local appearance of edematous bright red spots with borders, rapidly expanding in all directions. There is often a tendency for recurrence, and the symptoms are often milder when recurring. In infants, it is often seen on the abdomen and is associated with umbilical cord infection. There is a legacy of hyperpigmentation after healing. It needs to be differentiated from contact dermatitis and cellulitis. Clinical features] 1, etiology: group A beta hemolytic streptococcus caused 2, the first symptoms: chills, general malaise, high fever, 39 ~ 40 ℃ 3, the site of prevalence: calf, face. 4, lesion characteristics: bright red edematous spots, surface tension shiny, clearer borders, blisters may occur in severe cases Fig1. 5, systemic symptoms: local lymph node enlargement, fever, etc. 6, conscious symptoms: pressure pain is obvious, local skin temperature is high 7, prognosis: recurrent dengue causes chronic lymphedema, repeated attacks on the lower limbs may lead to elephantiasis. 8, experimental examination: total leukocyte count, neutrophilic leukocyte increase [differential diagnosis] contact dermatitis, cellulitis, ringworm rash [preventive treatment] treatment principles: active antibacterial, early, adequate and effective antibiotic therapy a, systemic treatment: antibiotic therapy, preferred penicillin, 4.8 to 8 million units / day IV, allergy can be used erythromycin ii, active treatment of local lesions such as ringworm, rhinitis, etc. Lower limbs should be elevated affected limbs. Third, supportive therapy, symptomatic treatment. Local treatment: furacilin liquid wet compress, topical antimicrobial ointment: such as Bactrim ointment, etc. V. Physical therapy: ultraviolet radiation, audio electrotherapy, ultra-short wave, infrared, etc. [Treatment of dermatophytosis] I. Western medical treatment of dermatophytosis 1. Sulfonamides can also achieve good results, and can be applied simultaneously with penicillin if necessary according to the condition. If the patient is allergic to penicillin, tetracycline and erythromycin can be used. If the patient has recurrent chronic dermatophytosis, he should be checked for tinea pedis in the toes and other places, and for infected lesions in the nasal vestibule and external ear canal, and be treated accordingly. The duration of antimicrobial drug application for recurrent dermatophytosis should be extended appropriately. Small dose X-ray irradiation, 50-100r (0.5-1Gy) each time, once every two weeks, 3-4 times in total, is also available. 2.Local treatment Elevation of the affected limb and topical antibiotic ointment are of little significance. The appropriate amount of hibiscus or dandelion leaves can be pounded and applied externally, or aluminum acetate solution, Revnur solution or horsetail decoction can be applied wet, which can reduce the degree of congestion and pain. How does Chinese medicine treat dermatophytosis? According to Chinese medicine, the cause of dermatophagoides is mainly fire toxicity, which can be caused by wind, dampness and heat evils turning into fire. If it occurs in the face, it is also called head-holding fire or big head plague; if it occurs in the lower limbs, it is called flowing fire; if it occurs in newborns or small children, it is called red swimming dan or swimming fire. The diagnosis and treatment can be divided into four types. (1) Wind-heat and fire incandescence is seen on the head and face, ears and neck, arms and shoulders, burning red, with heavy eyes closed and unable to open. It is accompanied by thirst and drinking, dry stool, red tongue, thin yellow coating, and slippery pulse. The main treatment is to disperse wind and clear heat and detoxify the toxin. (2) Stagnant fire in the liver meridian. It develops in the chest, abdomen, waist and back, dorsal ribs and umbilicus, with redness and swelling, expanding in all directions, with red tongue, thin yellow fur and string pulse. The treatment is to clear the liver and resolve dampness and heat. (3) Damp-heat and fire evidence It often occurs in the lower extremities, legs and feet, with redness, swelling and burning, spreading upward, enlarged lymph nodes in the groin, and difficulty walking. It is accompanied by a low level of nourishment, thirst and desire to drink, red tongue, yellowish greasy coating, and slippery pulse. The treatment is to clear heat and dampness and detoxify the toxin. (4) Toxic-heat-into-young evidence The scope of severe cases is larger, with delirium, restlessness, nausea and vomiting and other adverse symptoms. The treatment is to cool the blood and detoxify the toxin, clear the heart and open the orifice. Decoction with water, take 1 dose daily. For delirium, add Angong Niuhuang Wan, Zhi Bao Dan, Zi Xue Dan or Niuhuang Qing Xin Wan, choose one. Topical application of heat-clearing and detoxifying herbs can be used. In the early stage, use cactus, amaranth, hibiscus leaf, mung bean, etc., any one of them, and pound them externally, then change them if they are dry. In the middle and late stages, when the redness and swelling recede a little, you can use golden yellow paste or Ruyi golden yellow powder, honey water to dress. For recurring attacks of dengue, medicine can be taken to prevent it: 30g of raw barley, decocted once a day. During the treatment period, attention should be paid to rest and drink more warm water. And isolate from healthy people and avoid contact. Avoid eating spicy, fishy and greasy food, and eat more vegetables and fruits.
