Acute and chronic gastritis are acute and chronic inflammation of the gastric mucosa caused by various causes. Acute gastritis is an acute, reversible lesion of the gastric mucosa. Chronic gastritis is a common disease, the incidence of which is the highest among all kinds of gastric diseases, and the older the age, the higher the incidence. The disease can be caused by the transformation of acute gastritis, or by long-term exposure to gastric irritants or gastritis-causing bacteria. This disease belongs to the category of “gastric pain” in Chinese medicine.
I. Etiology and pathogenesis
According to Chinese medicine, acute gastritis is mostly a real disease, while chronic gastritis is a deficiency disease or a mixed disease of deficiency and reality. The stomach is the main recipient of and cooks water and grains, and the stomach may not function properly due to cold, diet, liver qi, or weakness of the spleen and stomach, causing stomach pain, nausea, vomiting, and other uncomfortable symptoms.
Second, the principle of treatment
According to the etiology and pathogenesis of acute and chronic gastritis, the basic treatment is to regulate qi and relieve pain. If the disease is obstructed, identify the evil and remove it; if the liver and stomach are not in harmony, dredge the liver and regulate the qi; if the diet hurts the stomach, eliminate the food and guide the stagnation; if the cold is in the stomach and the spleen and stomach are deficient in cold, warm the middle and disperse the cold.
III. Basic operation
Patient in supine position: push and massage the stomach and epigastric region for about 6 minutes; press and knead the middle epigastric region, Qihai, Tianshu, and the foot three li for one minute each; push from the upper epigastric region to Qihai with the thumb back and forth 5~6 times.
Patient in prone position: push the route of the bladder meridian on both sides of the back spine to Sanjiao Yu with one finger, 4~5 times round trip; press and knead Liver Yu, Spleen Yu, Stomach Yu, Sanjiao Yu and Large Intestine Yu for one minute each.
Patient sitting: take the shoulder well and follow the elbow of the arm, and do stronger stimulation at the hand Sanli, Neiguan and Hegu points; rub the shoulder and arm; rub and wipe the two hypochondrium from top to bottom, round trip 5~6 times.
Fourth, the identification and treatment
Cold evil offending the stomach: about 6 minutes of abdominal massage; rub the left side of the back (T7~12), in order to penetrate the heat; more focused, press the spleen Yu, stomach Yu for one minute each.
Dietary stagnation: focus on the central epigastric and tianshu, massage the abdomen in a clockwise direction for 5 minutes; press and knead Shenqu for one minute; press and knead Spleen Yu, Stomach Yu, Sanjiao Yu, Large Intestine Yu, the eight s, Foot Sanli, Liangmen, Zhangmen, Sliding Meat Gate and Gong Sun for one minute each.
Liver qi offending the stomach: push or knead Tiantu to Qihai for about two minutes with emphasis on Tanzhong; press and knead both sides of Zhangmen, Zhimen, Liver Yu, Gall Bladder Yu, Diaphragm Yu, Yanglingquan and Taichong for one minute each; rub both sides obliquely.
Weak spleen and stomach: gently press and knead Qihai, Guangyuan, and the foot Sanli for one minute each, and the treatment time for the Qihai point can be extended appropriately; directly rub the back of the Governor, and cross-rub the left side of the back (T7-12) and the waist of the Kidney Yu and Life Gate to penetrate heat; chiropractic 3 to 5 times.
For those with severe pain, first use heavier techniques to point and press the pressure points of Spleen Yu and Stomach Yu on the back and slightly pluck them, stimulate them continuously for two minutes, and pinch Neiguan for about two minutes, and after the pain is relieved, then apply the treatment according to the evidence.
V. Appropriate techniques
Chinese medicine: 3 slices of ginger, half a tael of brown sugar, decoction in water; two taels of fresh orange peel, one tael of vinegar, sugar mix.
External application: one catty of coarse salt, fried in wine and wrapped in cloth.
Acupuncture and moxibustion: main acupuncture points: Zhongshang, Neiguan, and Shusanli; supporting acupuncture points: cold evil offending the stomach with Stomach Yu (plus moxibustion); dietary stagnation with Liangmen; liver qi offending the stomach with Taichong; qi stagnation and blood stasis with Di Yu and Gongsun.
Sixth, precautions
1, the patient should have a regular life, diet, do not overeat, hunger and satiety, should eat less and more meals. Avoid tobacco, alcohol, spicy, sour or cold stimulants, and indigestible foods such as fried foods. If the pain persists, you should eat liquid or semi-liquid food for a certain period of time, and rest in bed.
2, regulate the mood, keep a cheerful mood; pay attention to rest, do not overwork.
3, gastric, duodenal ulcer bleeding period, or the stomach and epigastric muscle tension, prohibited manipulation treatment, so as not to aggravate the bleeding tendency.
4, long-term positive fecal occult blood, with wasting, low-grade fever and other symptoms, should be alert to malignant change.
