Traditional Chinese medicine treatment of ischemic optic neuropathy

Ischemic optic neuropathy is an acute obstacle to the blood supply of the arteries supplying the optic nerve, causing ischemia and hypoxia of the optic nerve, resulting in damage to the optic nerve, with onset in one or both eyes, and with intervals between the onset of both eyes. This disease occurs mostly in middle-aged and old people, but China reported that young patients also accounted for a certain proportion, and both men and women can develop the disease. Common causes: ① systemic vascular lesions: hypertension, nocturnal hypotension, atherosclerosis, diabetes, cardiovascular and cerebrovascular diseases, hyperlipidemia, carotid artery disease, severe anemia, as well as a variety of diseases that cause systemic hypotension such as post-partum or gastrointestinal tract acute hemorrhage, different causes of shock, intra-operative or post-operative drop in blood pressure, hemodialysis, cardiac failure and so on. ② Ocular causes: congenital inherent small optic disc and small optic cup, significant increase in intraocular pressure, optic disc buried glass warts, intraorbital or optic nerve sheath occupying lesions, endocrine-related eye diseases. ③ Other factors: homocysteinemia, rheumatism, severe eczema, oral contraceptives and so on. Cheng Juan, Ophthalmology Department of Traditional Chinese Medicine, Shanxi Provincial Eye Hospital Clinical manifestations: 1. Sudden decrease in visual acuity with some dark shadows in front of the eyes. 2. There is no obvious eye rotation pain, sometimes accompanied by headache, eye pain and supraorbital pain. 2. Visual field examination typically shows quadrant blindness and extends an arcuate dark spot to connect with the physiologic blind spot. There are also cases of early dark spot in the center or bow-shaped dark spot which further develops into half-blindness in the lower part of the eye. 3, fundus changes: visible optic papillae edema, edge unclear, optic papillae part or all of the color pale, edema bulge less than 3D, or normal color or congestion, disc periphery can be seen point, flame-like small hemorrhage. The retinal arteries often become thin and narrow. 4, Fundus fluorescence angiography early manifestation of optic disc ischemic area of non-fluorescence or weak fluorescence or filling retardation, retinal circulation is normal. If part of the ischemic area due to superficial capillary compensatory expansion leakage present strong fluorescence, so the optic disc on the obstruction of ischemic area and non-ischemic area fluorescence strength of asymmetry that is uneven phenomenon. 1 ~ 2 months after the occurrence of optic nerve atrophy. Traditional Chinese Medicine (TCM) attributes this disease to the category of “sudden blindness”, which is mostly characterized by qi stagnation and obstruction in the early stage of the disease, and in the late stage of the disease, it is mostly characterized by deficiency of qi and blood, and insufficiency of the liver and kidneys. Chinese medicine believes that the eye depends on the blood and qi to moisten and nourish, any external infection, internal injury, diet and fatigue, etc., can lead to malfunction of internal organs, qi and blood generation and operation disorders, the eye system is not enough to supply blood and the occurrence of this disease. The onset of this disease is fast, often involving both eyes, can be successive onset. If the disease is left untreated for a long time, it can lead to atrophy of the eye system. 1, qi stagnation and complex obstruction symptoms: eye disease with chest and hypochondrium distension and pain, dark red tongue with petechiae, stringy pulse as the main points of diagnosis. Treatment: promoting qi, activating blood circulation and opening up the collaterals. Prescription: adding and subtracting Easy San. 2. Qi and blood deficiency: loss of vision, pale nipple color, palpitation, fatigue, pale tongue and weak pulse as the main points of diagnosis. Treatment: benefiting Qi, nourishing blood and opening up the channels. Prescription: Tao Hong Si Wu Tang plus subtractions. Liver and kidney insufficiency: decreased visual acuity, reddish nipple color, lumbar pain and dystocia are the main points of identification. Treatment: tonifying the liver and kidney, regulating qi and clearing the channels. Prescription: Yi Yin Kidney Qi Pill plus reduction. This disease can be treated with acupuncture, acupoint injection and direct current ion implantation. A large number of clinical practices have proved that the combination of traditional Chinese and western medicine is effective in promoting the recovery of this disease and improving the visual function.
