Fiberoptic ductoscopy is increasingly valued as an important tool in the examination of breast duct disease. It provides an important basis for determining whether a patient with breast duct disease has a surgical pointer. The following are common conditions that require ductal lactoscopy.
1. patients with nipple overflow of various colors, especially those with bloody overflow and yellow overflow, the incidence of neoplastic lesions in the milk ducts is about 1/3 to 1/2, in addition to patients with white overflow which in some cases is due to a mass in the milk ducts, all of which require surgical treatment; Wu Jian, Department of Breast and Thyroid, Chengdu Third People’s Hospital
2. swelling in the areola area without nipple discharge, most of the swellings in this area are intraductal tumors or fibroadenomas, which are closely related to the milk ducts, and the diseased milk ducts can be clearly identified through ductoscopy, thus guiding the surgery to accurately remove the swelling and diseased milk ducts and reduce the chance of local recurrence after surgery.
3. Patients with plasmacytoid mastitis in the areola area and around the areola, plasmacytoid mastitis is a kind of dilated breast duct disease, because the proximal end of the milk duct is blocked, the distal end is dilated, the secretions and exfoliated cells in the milk ducts accumulate, and a large number of local plasma cells infiltrate, thus causing acute and chronic inflammatory reactions in the breast; lactoscopy can lavage and collect the exfoliated cells in the milk ducts, and perform cytological examination to clarify the diagnosis; at the same time, it can also In addition, if the inflammation is relatively limited, the diseased ducts can be identified under the ductoscope, and the diseased ducts and local scar tissue can be surgically removed.
4. patients with breast pain, as a type of breast enlargement, partly due to blockage of the proximal milk ducts, resulting in poor drainage and torsion of the distal milk ducts, lavage of the milk ducts under lactoscopy and unblocking of the milk ducts can help to clarify the diagnosis and hopefully achieve some therapeutic effect.
5. In patients with cumulative breast cysts, some patients can improve their symptoms after draining the milk ducts.
