1.Treatment principles Ovarian malignant tumors have different treatment plans depending on the type of pathology, and are mostly treated with surgery combined with chemotherapy and other comprehensive treatments. 2.Surgical treatment During surgery, detailed exploration should be performed first, including cytological examination of abdominal washings or peritoneal fluid, palpation of diaphragm, pelvic and abdominal organs, pelvic lymph nodes and retroperitoneal lymph nodes for accurate tumor staging. The surgical approach for early stage patients is divided into full staging surgery and staging surgery to preserve reproductive function. The scope of full staging surgery includes bilateral adnexal, uterine, and greater omentum resection and pelvic and retroperitoneal lymph node dissection. For advanced patients with extensive metastases in the pelvic cavity, tumor cytoreductive surgery is recommended whenever possible. Chemotherapy Since ovarian malignant tumors, especially epithelial carcinoma, spread very early, the lesions cannot be removed in most cases during surgery, and the effect and application of radiotherapy are also very limited, so systemic chemotherapy is an important adjuvant treatment method. Especially for malignant germ cell tumors, standardized chemotherapy can significantly improve the survival rate of patients. In some advanced patients, the mass can shrink after chemotherapy, creating favorable conditions for satisfactory tumor reduction during surgery. Radiation therapy The sensitivity of ovarian malignant tumors varies greatly. Endodermal sinus tumors, immature teratoma and embryonal carcinoma are the least sensitive, ovarian epithelial carcinoma and granulosa cell carcinoma are moderately sensitive, and asexual cell tumors are the most sensitive, which can be controlled mostly with radiation therapy after surgery. However, due to the fact that malignant germ cell tumors such as asexual cell tumors are mostly adolescents and chemotherapy is effective, and the side effects of abdominopelvic radiotherapy are large, radiotherapy has rarely been used for ovarian malignant tumors.
