How to diagnose and treat appendicitis during pregnancy

  Appendicitis is more common during pregnancy. Especially in the middle of pregnancy, the uterus enlarges faster, the appendix and appendix are pushed by the enlarged uterus to the right upper abdomen, and the site of pressure pain is also shifted upward. The abdominal wall is elevated and the inflamed appendix does not stimulate the mural peritoneum, so that the pressure pain, myalgias and rebound pain are not obvious; the large omentum has difficulty in wrapping the inflamed appendix, and peritonitis is easily confined and spreads in the abdominal cavity. These factors make it difficult to diagnose acute appendicitis in mid-pregnancy, and the inflammation is likely to cause miscarriage or preterm delivery, threatening the life of mother and child. Early appendectomy is the mainstay of treatment. In late pregnancy, abdominal infection is difficult to control, so early surgery is even more important. Progesterone should be added during the perioperative period. In acute appendicitis in the perinatal period, if the appendix is perforated or the symptoms of systemic infection are severe, transabdominal caesarean section can be considered and the diseased appendix can be removed at the same time.
