Advantages of minimally invasive treatment of appendicitis

  Should appendicitis be treated minimally invasively using laparoscopy?  I believe there are still many general surgeons who have inconsistent views. Some colleagues believe that minimally invasive surgery is expensive, the surgical preparation process is slightly more complicated, more instruments are needed, and some hospitals do not have skilled instrument nurses to work with on the night shift in the operating room, so the night shift does not allow doctors to use laparoscopy. The more mainstream view is that appendicitis can be completely removed with small incisions, at low cost and in a short time, and some general surgeons have limited proficiency in laparoscopic operation.  Advantages of minimally invasive laparoscopic treatment: 1. There are various difficult situations in the clinic when traditional open surgery to remove the appendix is done. For example, in particularly obese patients, the appendix has been septic and perforated, the incision infection rate is very high, once the incision is infected and cracked, the resident needs to change the medicine every day, the patient has to stay in the hospital for one to two more weeks, and some need to close the wound again. In this case, the advantage of laparoscopy is very big, because there is no surgical incision, only a 1cm puncture hole, there is almost no chance of infection.  2, laparoscopic surgery can easily explore the whole abdomen once, and the abdominal organs of the patient can be examined completely during the surgery, and the abdominal cavity can be examined under direct vision, which is a good opportunity for the patient to have a physical examination.  3, lies in the management of difficult appendix, the so-called difficult appendix surgery, one is the difficulty of finding the appendix, for low senior doctors in the traditional small incision open surgery to find the ectopic appendix will be very difficult, sometimes have to extend the surgical incision, resulting in increased surgical trauma, and surgical incision complications (such as incisional hernia). There are also operational difficulties, although it is a minor surgery, there are also difficult cases, while laparoscopy is operated under direct vision, the laparoscope has the effect of magnification, and the surgical procedure is much easier.  4. The chance of intestinal obstruction with abdominal adhesions is reduced. Laparoscopy under direct vision can clean the abdominal cavity more cleanly and suck out the pus, and there is no direct contact with the hand to pull the intestinal tube, and the intestinal tube is not directly exposed to the air, all of which can reduce the occurrence of intestinal adhesions and greatly reduce the rate of surgical complications. In conclusion, the advantages of minimally invasive surgery are becoming more and more obvious, and with the development of surgical techniques, laparoscopic surgery will gradually replace traditional surgical methods. Laparoscopic appendectomy uses an electric knife to cauterize the appendiceal mesentery and appendiceal vessels sufficiently, and the use of metal or plastic clips can leave foreign bodies. The appendiceal stump is cauterized with an electric knife without suturing and embedding. It is better to close the stump reliably with ultrasonic knife and ligating the root with 7 gauge silk is sufficient.
