Don’t talk about hormones in bronchial asthma

Asthma is a common disease that jeopardizes people’s health, and is currently one of the fastest growing diseases in the world, causing a social burden that exceeds the sum of AIDS and tuberculosis, with the worldwide incidence of asthma increasing by 50 percent every 10 years on average. Currently, there are about 300 million asthma patients in the world and 25 million in China. Regular inhaled hormone therapy is the most commonly used asthma treatment method, which has been more and more widely used, but there are still some patients who are still skeptical about inhaled hormone therapy, worrying that inhaled hormone will have side effects on their body, in addition, for inhaled hormone therapy regular treatment program is also not very well understood, which affects the efficacy of the treatment. 1, irregular medication in remission is an important reason for severe asthma attacks and even sudden death Asthma is a chronic recurrent disease that requires long-term treatment. Some patients only take emergency means, only in the asthma attack to remember the treatment, and in the remission period does not use any drugs, so repeated attacks, over time will cause irreversible serious damage to the patient’s lung function, leading to emphysema, pulmonary heart disease and other serious complications. Irregular use of medication during the remission period is also an important cause of severe asthma attacks and even sudden death. The nature of bronchial asthma is a variety of cells and cytokines involved in chronic airway inflammation, this inflammation belongs to the metabolic inflammation, unlike bacterial infectious inflammation, the application of antimicrobial therapy is ineffective, only the regular application of hormones can inhibit such inflammation. The modern treatment of asthma should focus on the period of remission, through regular inhalation of hormones, to achieve the purpose of suppressing airway inflammation, maintaining the patient’s normal or more or less normal lung function, preventing asthma attacks, and ensuring the patient’s quality of life. It is necessary to adhere to the regular use of medication, so as to extend the period of asthma relief, minimize the number of episodes, even if there are acute episodes, but also to reduce the degree of attacks. In short, the principle of asthma treatment should be “long-term treatment”. 2, long-term inhalation hormone therapy will cause systemic side effects long-term inhalation hormone will produce some patients worried about “side effects”? Prolonged systemic application (such as oral, intravenous infusion) hormone does have some side effects, may cause centripetal obesity, elevated blood glucose, hypocalcemia, osteoporosis and even femoral head necrosis. After taking hormones orally, the drugs are absorbed through the gastrointestinal tract and distributed to all tissues and organs of the body through blood circulation, and the long-term application of the above side effects may occur. This creates a contradiction, on the one hand, asthma patients need to apply hormones for a long time, on the other hand, long-term oral hormones will have serious side effects. In order to solve this contradiction, inhaled hormone therapy came into being, through the inhalation of hormone therapy, the drug is directly inhaled into the trachea, directly acting on the airway, the dose of the drug absorbed into the blood circulation is very small, the dose of hormone needed for inhalation hormone therapy is also much smaller than the dose needed for oral administration, which is equivalent to about 1/10 to 1/20 of the oral dose, so long-term inhalation of hormone therapy will not cause serious systemic side effects. Therefore, long-term inhaled hormone therapy does not cause serious systemic side effects. Only a small proportion of patients (about 2-3%) may experience mild reactions such as mouth ulcers, hoarseness, sore throat, etc. As long as attention is paid to gargling after using the drug, the above reactions will be reduced or disappeared. There are also a very small number of patients who may develop symptoms such as hirsutism. On the contrary, due to the lack of understanding of the characteristics of inhaled hormone therapy, overly worried about the so-called “side effects”, in remission without any drugs, so that asthma recurrent episodes, over time there will be airway remodeling, so that the patient’s lung function is irreversibly serious damage, the loss of optimal treatment, it will be too late to regret. 3, wheezing symptoms, reduce the dose of hormones on their own is also very wrong There are some asthma patients, in the inhalation of drugs 1-2 weeks, conscious wheezing symptoms significantly reduced, on their own to reduce the dose of hormones, this practice is also very wrong, because asthma is a chronic airway inflammation, airway inflammation control is a process, wheezing symptoms do not mean that airway inflammation has been well controlled, if you are in the airway inflammation control, you should be aware of the symptoms of asthma. The reduction of wheezing symptoms does not mean that the airway inflammation has been well controlled, and if the dose of inhaled hormones is hastily reduced at this time, it is easy for asthma to flare up. According to the asthma guidelines, a certain dose of hormone inhalation therapy should be used to achieve no significant acute exacerbation, the peak flow rate is basically normal, and should be maintained for at least 3 months before considering reducing the dose of hormone, and it often takes a year or even a few years from the start of the application of a larger dose of inhalation therapy to the application of the smallest dose of maintenance therapy in a particular patient with asthma. Only by taking the medication as prescribed by the doctor, having regular checkups, and adjusting the type and dose of medication under the doctor’s guidance and in strict accordance with the asthma treatment guidelines, can we achieve long-term good control of asthma and truly achieve “healthy breathing every day”.
