Si Miao Wan is composed of Atractylodes Macrocephalae, Phellodendron Bark, Radix Achyranthes Bidentatae and Coix Coix lacryma. Among them, Atractylodes Macrocephala and Phellodendron can clear heat and dry dampness; Bupleurum can strengthen tendons and bones and induce the downward movement of medicine; Job’s Tear has the effect of strengthening the spleen and relieving dampness, clearing heat and removing paralysis. The four herbs used together have the effect of clearing heat and promoting dampness and strengthening tendons and bones, and can be used for the treatment of joint pain in the lower limbs, soreness and weakness of the waist and knees, or redness and pain in the local joints caused by damp-heat infiltration, and also for the treatment of damp-heat banding and itching of the vulva in women, dampness and itching of the scrotum in men, and eczema of the lower legs. It is commonly used in modern TCM clinics for the treatment of rheumatic and rheumatoid arthritis, scrotal eczema, vaginitis, arteriosclerosis of the lower extremities, thrombophlebitis, diabetic foot, and other TCM conditions belonging to damp-heat infiltration. However, because the overall composition of the drug is on the cold side, it is not recommended for people with a weak spleen and stomach or a cold spleen and stomach.
