When is a prostate puncture biopsy necessary?

Generally, prostate puncture biopsy should be performed if one of the following conditions occurs and there are no contraindications to prostate puncture: (1) Prostatic nodules palpated on rectal examination; (2) Ultrasound findings of hypoechoic nodules in the prostate or abnormal signals on MRI (3) PSA >10 ng/ml; (4) 4 ng/ml < PSA <10 ng/ml and the ratio of free PSA to total PSA ratio is less than 0.16. For patients with negative prostate results after initial prostate puncture, PSA levels and prostate ultrasound and MRI need to be reviewed regularly. For patients whose PSA is still elevated during follow-up or whose imaging is still abnormal, repeat prostate puncture biopsy is recommended, usually chosen at about 1-3 months after puncture.
