Abstract: This case of a 1-month-old child presented with a rash on the buttocks and perineum area, crying and restlessness, and was brought to the clinic by the parents. It was considered to be a consequence of diaper dermatitis, and Chinese herbal treatment was given, including Phellodendron, Poria, Goldenseal, Bitter ginseng, Dioscorea, Amaranthus, and Comfrey, with topical external washing of the patient’s area and topical application of ointment, which had a good effect of clearing heat and relieving dampness and itching, and the child’s condition was significantly relieved.

I. Initial consultation
The child showed a series of symptoms such as redness of the skin on the buttocks and perineum area, sleeplessness at night, crying and restlessness. We asked the parents about their usual care and learned that the child was usually taken care of by her mother at home, so we mainly considered that the diaper dermatitis was caused by untimely diaper changes and local impermeability. On examination, we found that the skin of the child’s buttocks and perineum were obviously red, and there was a little fluid exudation without obvious pus or abnormal odor. The child’s history of inappropriate care, combined with the localized specific lesions of redness of the skin on the buttocks and perineum, was considered to be diaper dermatitis.
II. Treatment history
Given that the child’s condition was not particularly serious, intervention was carried out using Chinese herbs, which can be decocted at home and washed externally. The ingredients of the herbal medicine include Phellodendron, Poria, Ginkgo biloba, Ginseng, Radix et Rhizoma, Amaranthus, and Comfrey, which together can play a good role in clearing heat, relieving dampness, and relieving itching, together with Qingpeng ointment applied topically to improve diaper dermatitis. The parents also paid attention to the local skin care of the child at home, giving timely cleaning after stool and urine, and taking medication on time.
Third, the effect of treatment
After 1 week of intervention with Chinese herbal medicines such as Phellodendron, Fu Ling, Jin Yin Hua, Bitter Ginseng, Dioscorea, Amaranthus, and Comfrey, and topical application of Qing Peng cream, the child’s condition improved significantly, the redness of the skin on the buttocks and perineum area basically subsided, and there was no more inflammatory fluid exudation.
IV. Notes
We are glad that the child’s condition has improved significantly after treatment. Most of the diaper dermatitis is due to the young age of the child, the skin is very delicate, coupled with the parents do not have good care, such as not timely cleaning after urination and defecation, infrequent diaper changes, etc.. It is recommended that parents should pay attention to strengthening the skin care of their children, such as timely local cleaning with warm water after urination and defecation, and changing diapers. In addition, it is important to follow the doctor’s instructions and adhere to the medication, most of which can effectively improve the problem of red skin on the buttocks and perineum. In case of breastfeeding, the mother herself should pay attention to a proper light diet and eat less cold, greasy and spicy stimulating food to avoid aggravating the symptoms of the affected child. Because of the delicate skin of children, the probability of diaper dermatitis occurring clinically is very high, so the practical value of finding a treatment for diaper dermatitis in children is very high. In addition to the application of topical ointments, the use of Chinese herbal medicine can often be twice as effective as the application of topical ointments, for example, in this case, the application of Phellodendron, Poria and other Chinese herbal medicines and the application of Qingpeng ointment, so that the child’s condition can be effectively improved.
