How to implant a lens for congenital cataract?

  Due to the lack of a lens after congenital cataract surgery, the baby’s eye is highly hyperopic and needs to be optically corrected as soon as possible so that the retina can be stimulated with clear objects and the baby’s visual system can develop like a normal child of the same age. Is it better to implant an intraoperative lens in one stage or to wait until the baby is older and implant a lens in the second stage? When is the right time to implant a lens?  The results of a one-year controlled study conducted by the 2010 U.S. Study of Infant Aphakia in 114 children younger than 7 months with monocular congenital cataracts showed that there was no significant difference in visual acuity between children with phase I implants and those with postoperative contact lens correction; however, children with phase I implants required surgery due to complications associated with the lens. However, the rate of reoperation is significantly higher in children with stage I implants than in children with contact lens correction because of complications associated with the lens, which require surgical removal of the pupillary area of the augmentation membrane or the posterior cataract. Therefore, U.S. experts recommend that children under the age of 6 months should be careful with first-stage implantation.  The American Association of Pediatric Ophthalmology and Strabismus Specialists in 2001 used to recommend the time of lens implantation to be (1.4±1.4) years for children with monocular cataracts and (2.2±2.2) years for children with bilateral cataracts. At present, the general consensus among cataract specialists in China is that for children with cataracts found within 1 year of age, cataract removal surgery should be performed as early as possible to ensure normal development of the visual system and to correct vision through glasses after surgery, and IOL implantation should be done in the second stage after 2 years of age; whereas for children with cataracts found above 2 years of age, IOL implantation can be done at the same time as cataract removal.
