Trigeminal neuralgia case: a complete solution to the “number one pain in the world”

Trigeminal neuralgia is known as the “number one pain in the world”. The pain can be severe when talking, washing the face, brushing the teeth, or when walking in the breeze. The pain lasts for a few seconds or minutes, the number of episodes is variable, and the intervals are asymptomatic. The pain is most pronounced in the cheeks, upper and lower jaws and tongue, and can be triggered by light touch on the nose, cheeks and tongue, called trigger points. The pain is periodic and the interval between attacks is the same as normal. It really hurts like hell! So what else can be done for trigeminal neuralgia besides taking medicine? Ren was admitted to the hospital mainly because of “recurrent click-like pain on the left side of the face for more than 10 years”. The main clinical manifestation was recurrent clicking-like pain on the left side of the face, which was effectively treated with oral carbamazepine. Auxiliary examination: cranial MRI brain parenchyma did not show significant abnormalities, excluding intracranial occupying lesions. The preliminary diagnosis was 1. trigeminal neuralgia and 2. chronic bronchitis. After Ren was admitted to the hospital, he completed all the examinations and preoperative preparations, and all the examinations were not contraindicated. The surgical result was good. The right facial pain disappeared after the operation, and the postoperative recovery was good. Dr. Wang, the chief physician, instructed that the patient could be discharged after checking the room. At the time of discharge, Ren was in good condition, with a clear consciousness and mental status. He was eating and sleeping well, and his bowel movements were normal. General activity was good. The pain in the right side of the face disappeared. Head incision healing grade A/class I. Trigeminal neuralgia is a very common neurological disease of the brain, mainly manifested by recurrent paroxysmal severe pain in the distribution area of the trigeminal nerve on one side of the face, with more female than male patients, and the incidence may increase with age. Trigeminal neuralgia occurs mostly in middle-aged and elderly people, with the right side more often than the left. The disease is characterized by sudden onset, sudden stop, lightning-like, knife-like, burning-like, intractable and severe pain in the trigeminal nerve distribution area of the head and face. Trigeminal neuralgia harms: 1, affect the life and work of people, the most direct is to affect the sleep, light sleep difficulties, the heavy sleeplessness. 2. Affects people’s psychological health. This is because when people suffer from headache disease for a long time, their personality changes and they tend to become irritable. And because of the long time of treatment, life is greatly affected, psychological fragility and loss of confidence. ”Microvascular decompression” is a treatment method that targets the cause of trigeminal neuralgia, separating the compressed blood vessels from the nerve, treating trigeminal neuralgia from the root, and preserving the anatomical integrity of the trigeminal nerve. Therefore, the normal nerve function of the trigeminal nerve can be preserved. Some patients can also eliminate the state of hypertension caused by vascular compression of the brainstem and achieve the purpose of treating hypertension. At present, it has been recognized by the majority of patients.
