In recent years, there are more and more patients with facial spasm, trigeminal neuralgia and glossopharyngeal neuralgia, and the majority of them are middle-aged and elderly, and the proportion of patients with the above three diseases choosing surgical treatment is also increasing, but there are still some patients who choose to endure for a long time because of the following reasons: 1. In view of the above, we suggest that patients with the above three diseases should go to the neurosurgery department of a large and trustworthy hospital when they are at a loss. It is believed that there will be a effect of “the mountain will be overwhelmed, and the village will be clear again”. The following is an introduction to our work: our work as surgeons is intense and regular, mainly outpatient and surgical. Outpatient work two days a week: 1, to make a correct diagnosis of patients; 2, one-on-one analysis of the condition, to answer the patient’s questions, to eliminate the patient’s concerns; 3, for patients to develop individualized treatment plan and the best treatment time, so that patients no longer blind treatment, less detours. Three days a week surgical work: Microvascular decompression is the only method that can cure trigeminal neuralgia, facial muscle spasm and glossopharyngeal neuralgia at present, and it is also the only method to treat the cause of the disease. Let’s take a look at what microvascular decompression is: Microvascular decompression uses a small curved incision behind the ear to open a bone window about the size of a $1 coin, apply microsurgical techniques to separate the blood vessels and perineural adhesions, remove the responsible blood vessels compressing the nerve, and release the compression of the nerve by the blood vessels, thus achieving the purpose of treating the above three diseases. Microvascular decompression is completely operated by using the natural cavity in the brain, without damaging or destroying brain tissue and cranial nerves, preserving the integrity and physiological function of nerves, minimally invasive, safe and fast recovery. Is the efficiency of microvascular decompression surgery for the above three diseases high? At present, the efficiency rate of different centers at home and abroad is between 80-95%. I feel that the reason why there is such a big difference in the treatment effect is due to the following three factors: 1. correct diagnosis: the prerequisite for good treatment effect must be the correct diagnosis of the disease, and we should make adequate differential diagnosis to avoid misdiagnosis. 2, perfect examination: preoperative cranial nerve thin scan MRI is essential. 3.Individualized surgical plan: The surgical operation cannot be patterned, and the surgical plan should be made according to the actual situation of the patient’s nerves and blood vessels observed intraoperatively, including the nerve exposure, arachnoid freeing, the path of pad cotton placement and the pad cotton fixation position. We first proposed the concept of vascular grooming in microvascular decompression surgery, focusing the surgical operation on the responsible vessels, and we always had to make it clear that the purpose of the procedure was nerve decompression and the essence was vascular grooming. Thanks to the above-mentioned surgical concept, together with the correct diagnosis of the disease and the perfect preoperative examination, we have achieved a surgical efficiency of over 95% in recent years. After nearly 20 years of exploration and accumulation, microvascular decompression surgery has become the characteristic treatment of neurosurgery in Shandong Provincial Hospital, and the surgical effect is among the top in China.
