Blockage of bile duct stones can lead to impaired bile evacuation and also to secondary infection, which in severe cases can lead to acute severe obstructive purulent cholangitis and can be life threatening. In this case, surgery should be performed in a timely manner to remove the stones from the common bile duct to relieve the obstruction, and also to explore the bile duct, which can be done by choledochoscopy. The gallbladder should also be removed intraoperatively, as patients with bile duct stones are mainly caused by peristaltic impaction of gallbladder stones in the bile duct. Postoperatively, a T-shaped drain is also left in place to drain the bile outside the body. Post-operative symptomatic treatment such as intravenous anti-infection therapy and intravenous nutritional support is also required. Patients are also advised to leave bed early and to stay away from stimulating foods and high-fat foods during the recovery period.
