“Biliary care” is an important project after biliary lithotripsy

The gallbladder is an important organ of the human body, and all doctors who can perform minimally invasive endoscopic biliary stone removal will remove the stones and polyps from the gallbladder and preserve the gallbladder intact, as long as the gallbladder has not been organically and functionally irreversible damaged, that is, as long as the gallbladder still has function or function can be restored. However, surgery is only a symptomatic treatment. To cure the problem, the function of the gallbladder must be protected for a long time to avoid the recurrence of stones. Gallbladder protection is a long-term and important project, which includes the following aspects. 1, regular review of gallbladder function within six months after surgery, generally once in 1, 3 and 6 months after surgery to receive liver and gallbladder ultrasound examination to understand the gallbladder and its functional status. The purpose of surgery is to avoid obstruction and inflammation caused by stone polyps and their damage to gallbladder function, while the stimulation and damage to gallbladder caused by surgery itself will have different degrees of impact on gallbladder function. The degree of impact varies from patient to patient, with some recovering more quickly and others more slowly. It has been proved that many patients need half a year to fully recover after surgery, and if they do not pay attention to gallbladder protection during this half year, the gallbladder will be poorly emptied and the bile will be easily stagnated. Therefore, it is often necessary to cooperate with the doctor during this period. If necessary, take some medication. 2, bile care medication, the patient 3-5 days after surgery until 3-6 months after discharge from the hospital, the doctor will, depending on the nature of the patient’s stones and the state of the patient’s gallbladder function after discharge, give the patient some drugs to promote the emptying of the gallbladder and/or increase the secretion of bile acids and change the proportion of bile composition to prevent stone formation. Recent studies have found more drugs to promote gallbladder emptying and prevent stone recurrence, such as mosapride, cisapride, erythromycin, indolamine, rhodopsin, trimebutine maleate, ursodeoxycholic acid, taurodeoxycholic acid, and anti-inflammatory and cholagogic tablets, and many other Chinese medicines or Chinese medicine prescription drugs. The pharmacological effects of different drugs are different, and the side effects of different drugs for different treatment targets are also different. According to the specific situation of the patient, under the guidance of the doctor to choose. 3, diet and exercise, not all patients need to take long-term medication after biliary lithotripsy, to promote the gallbladder contraction and emptying, diet structure and regular regulation is more important. Milk and fatty foods can promote the contraction and emptying of the gallbladder, and it is a misconception to avoid oil and meat after gallbladder stone surgery. The gallbladder needs a lipid diet to contract and empty. A high-fat, high-cholesterol diet is prone to grow gallbladder stones, while a non-fat vegetarian diet, without contraction and emptying of the gallbladder, is prone to gallbladder inflammation and bile stagnation into stones. Therefore, it is recommended that the lipid diet of postoperative gallstone patients should be based on vegetable fats, including walnuts, nuts and other fatty nuts, to minimize the intake of animal fats and cholesterol. In addition, exercise can increase lung capacity and increase intra-abdominal pressure. The repeated increase in intra-abdominal pressure causes compression of the gallbladder, which also has a role in the emptying of the gallbladder. The gallbladder is a useful organ and preserving a functioning gallbladder is beneficial to one’s health, but requires a concerted effort by both the doctor and the patient.
