In the majority of cases, the vagina is replaced by solid tissue, the uterus is underdeveloped or has a solid base, the ovaries may be in a different position but are functioning normally, the chromosome is 46XX, so there are obvious female secondary sexual characteristics and sexual physiology, the perineal structures such as the distribution of pubic hair, the clitoris, the labia majora and minora are normal, and the body shape and breast development have female characteristics. In rare cases, only the vagina is absent, but the uterus, ovaries and adnexa are well developed. They are socially discriminated against and suffer from mental depression, and when they reach adulthood, they ask for vaginal reconstruction in order to start a family or restore their fertility. This type of person is commonly known as a “stone girl”. There are various methods of vaginoplasty, including die top compression; skin grafting; peritoneal grafting; amniotic membrane grafting; flap grafting; and intestinal segment grafting. Currently, the main focus is on flap grafting, but its destruction of the vulva, scar formation, and growth of hair in the cavity has been a headache for surgeons. With the increasing maturity of abdominal gastrointestinal anastomosis technology, the intestinal graft method of vaginoplasty has achieved more satisfactory results. Sigmoid graft vaginoplasty has been reported in many cases in China, but ileal graft vaginoplasty is less reported. The vagina formed by sigmoid graft will secrete a lot of smelly mucus and vaginal bleeding at the beginning of sexual life will disappear completely after several months. Jiangsu Provincial Hospital of Traditional Chinese Medicine has successfully performed laparoscopic assisted ileal graft vaginoplasty with vascular tip for 7 “stone women” and 2 transgender patients, in which the vagina is deepened and widened by first removing the vaginal flap with hair that was previously done, and then the vagina formed by the ileum cut under laparoscopy is pulled down. The “vagina” is then pulled out of the body and sutured to the cavity opening to form a new vagina. This method overcomes the disadvantages of dryness and atrophy of skin, amniotic membrane and peritoneum, the need to wear molds for a long time after surgery and the large scars after flap surgery, and the disadvantage of odor of sigmoid colon, and it is an ideal method of artificial vaginal reconstruction surgery with realistic shape, little trauma, little bleeding and fast recovery, and currently this kind of technology is in the leading position in the province.
